THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) on her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show a third award of the Meritorious Service Medal; and that item 14 (Military Education) on her DD Form 214 be corrected to show she completed the Emergency Medical Technician Basic Course, the Practical Nurse Course, and the Doctrines and Tactics Training Course.

2.  The applicant states, in effect, that the training certificates were in another state in storage during the processing of her DD Form 214 and that the award for the Meritorious Service Medal was not yet presented when her DD Form 214 was processed.  

3.  The applicant provides certificates of training for the Emergency Medical Technician Basic Course and the Doctrines and Tactics Training Course; orders for the Meritorious Service Medal, a diploma for the Practical Nurse Course; and a copy of her DD Form 214. 

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 12 May 1987.  She served as a health care specialist and remained on active duty through continuous reenlistments.  On 31 December 2008, she will be retired in the rank of sergeant first class.

2.  Item 13 on the applicant's DD Form 214 does not show a third award of the Meritorious Service Medal as an authorized award.  Item 14 on her DD Form 214 shows she completed the 44-week Practical Nurse Course in 1991.  Since the Practical Nurse Course is shown on the applicant's DD Form 214, this portion of her request will not be discussed further in these Proceedings.  

3.  Item 14 on the applicant's DD Form 214 does not show that she completed the Emergency Medical Technician Basic Course or the Doctrines and Tactics Training Course.

4.   The applicant provided a certificate of training which shows she completed the 120-hour Emergency Medical Technician Basic Course during the period 
30 October 2000 to 22 November 2000.

5.  The applicant provided a certificate of training which shows she completed the Doctrines and Tactics Training 40-hour Certification Course during the period 
8-12 May 2006.

6.  Headquarters, 32nd Army Air and Missile Defense Command Permanent Order Number 261-01, dated 17 September 2008, shows the applicant received the third award of the Meritorious Service Medal for the period 1 January 1999 to 31 December 2008.  These orders also show that her previous awards included two awards of the Meritorious Service Medal. 

7.  Army Regulation 635-5 establishes the standardized policy for preparing and distributing the DD Form 214.  The regulation provides that item 14 will list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by title, length in weeks, and year completed.  

DISCUSSION AND CONCLUSIONS:

1.  Orders show the applicant received the third award of the Meritorious Service Medal.  Therefore, her DD Form 214 should be corrected to show this medal.

2.  Evidence of record shows the applicant successfully completed the 120-hour (3 weeks) Emergency Medical Technician Basic Course in 2000 and the Doctrines and Tactics Training 40-hour Certification Course in 2006.  Therefore, it would be appropriate to correct item 14 on her DD Form 214 to show these courses.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  adding the third award of the Meritorious Service Medal to item 13 on her DD Form 214; and

	b.  adding the entries, "Emergency Medical Technician Basic Course, 3 weeks, 2000" and "Doctrines and Tactics Training Certification Course, 1 week, 2006" in item 14 on her DD Form 214.
