THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to add military occupational specialty (MOS) 91C (Practical Nurse) to item 11 (Primary Specialty) and the Practical Nurse Course to item 14 (Military Education).
2. She states she believes the MOS was inadvertently omitted from her
DD Form 214 because item 14 does show she completed the required training.
3. She provides:
* A course completion certificate for MOS 91C
* Her DD Form 214
* An Army/American Council on Education Registry Transcript
CONSIDERATION OF EVIDENCE:
1. The applicant's initial enlistment documents are not available for review; however, her DA Form 2-1 (Personnel Qualification Record Part II) shows the following information:
a. Item 17 (Civilian Education and Military Schools) she completed
16 weeks of advanced individual training (AIT) for MOS 91D (Operating Room Specialist) in 1987.
b. Item 35 (Record of Assignments) shows she had 6 months and 7 days of prior service as an Operating Room Specialist at Fort Eustis, VA. She enlisted in the Regular Army on 11 August 1988 and served in MOS 91D for a total of 4 years, 10 months, and 25 days.
2. She provided a copy of a course completion certificate which shows she successfully completed the 55-week Practical Nurse Course for MOS 91C at Fort Gordon, GA. This certificate shows the course began on 6 July 1993 and ended on 29 July 1994.
3. Item 35 of her DA Form 2-1 also shows she served in MOS 91C from
24 August 1994 through 4 December 2003 for a total of 9 years, 3 months, and
11 days. Her highest grade during this period of service was staff sergeant (SSG)/E-6.
4. The applicants Enlisted Records Brief (ERB) indicates she served the remainder of her service at Fort Sam Houston, TX in various positions in MOS 91W, which later converted to MOS 68W.
5. On 29 February 2008, she honorably retired after completing more than
20 years of active military service. Item 11 of this form shows the following entries:
* 68W4O Health Care Specialist 19 Years, 6 Months
* 68D4O Operating Room Specialist 13 Years, 6 Months
6. Item 14 of her DD Form 214 does not show she completed the 55-week "Practical Nurse Course."
7. The Army G1, Personnel Authorization Module (PAMXXI), which is located on the Army G1 portal, is the force structure portal for the Army. This document provides information on force structure, unit manning, Army demographics and MOS charts with conversion data. It shows that MOS 91C converted to MOS 91W (Health Care Specialist) on 1 October 2001. It further shows that MOS 91W converted to MOS 68W on 1 October 2006.
8. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The regulation provides that the Primary MOS and all additional MOS served in, for a period of one year or more, during the Soldiers continuous active military service, will be entered in item 11. For each MOS, list the title with the years and months served. For time determination, 16 days or more count as a month and that basic combat training (BCT) and advanced individual training (AIT) are not counted.
9. The same regulation states to list in-service (full-time attendance) training courses successfully completed during the period of service in item 14 of the
DD Form 214. This information is to assist the member after separation in job placement and counseling; therefore, do not list training courses for combat skills.
DISCUSSION AND CONCLUSIONS:
1. Her request to have MOS 91C added to item 11 and the Practical Nurse Course to item 14 of her DD Form 214 was carefully considered.
2. MOS 91C converted to MOS 91W on 1 October 2001. MOS 91W later converted to MOS 68W on 1 October 2006. Both conversion periods were prior to the applicants retirement from the Army and accordingly, her MOS was converted during each period. Therefore, the MOS currently depicted in item
11 of her DD Form 214 is correct.
3. Her records also show she successfully completed the 55-week course for Practical Nurse and was awarded the MOS 91C. As a result she is entitled to have this course added to item 14 of her DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by correcting her DD Form 214 for the period ending 29 February 2008 by adding in item 14 Practical Nurse CRS, 55 Weeks, 1994//.
2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to adding the MOS 91C to item 11 of her DD Form 214.
