THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show completion of the Practical Nurse Course. 

2.  The applicant states, in effect, she completed the course in 2010 but it was omitted from her DD Form 214. 

3.  The applicant provides an Academy of Health Sciences Diploma and her 
DD Form 214. 

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 1 August 2002 and she initially held military occupational specialty (MOS) 42A (Human Resources Specialist).  She served through multiple reenlistments and she attained the rank/grade of staff sergeant/E-6. 

2.  In November 2006, she attended and successfully completed the training requirements for award of MOS 68W (Health Care Specialist). 

3.  Her records contain:

	a.  DA Form 1059 (Service School Academic Evaluation Report) confirming her attendance of the Health Care Specialist Course at the Academy of Health Sciences, Fort Sam Houston, TX, from 31 July to 22 November 2006, a period of 16 weeks.  

	b.  DA Form 1059, and she provides a diploma from the Academy of Health Sciences, Fort Sam Houston, TX, confirming completion of the Practical Nurse Course from 16 March 2009 to 24 March 2010, a period of 53 weeks. 

4.  She was honorably discharged on 13 July 2014 to accept a commission.  Her DD Form 214 shows she completed 11 years, 11 months, and 13 days of active service.  Item 14 (Military Education) of this form does not list the Practical Nurse Course. 

5.  Army Regulation 635-8 (Separation Processing and Documents) states item 14 will list all formal, in-service (full time attendance) training courses successfully completed during the period of service covered by the DD Form 214 of at least 1 week or 40 hours duration.  Include course title, length in weeks, and year completed.  This information is to assist the Soldier in job placement and counseling; therefore, do not list training courses for combat skills.  Acceptable source documents include the enlisted record brief, DA Form 4037 (Officer Record Brief), DA Form 1059, or other military issued certificate of completion with from and through dates or number of weeks.

DISCUSSION AND CONCLUSIONS:

The applicant successfully attended and completed the 53-week Practical Nurse Course in 2010.  Her DD Form 214 inadvertently omitted this course.  She should be entitled to correction of her DD Form 214 to add this course.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to item 14 of her DD Form 214 the entry "Practical Nurse Course, 
53 weeks, 2010."
