      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that Item 4a (Grade, Rate, or Rank),

Item 4b (Pay Grade), and Item 14 (Military Education) of her separation
document (DD Form 214) be corrected.

2.  The applicant states, in effect, that Items 4a and 4b of her DD Form
214 should be corrected to show that she held the rank of staff sergeant/E-
6
(SSG/E-6), at the time of her discharge.  She also states that her
separation document should be corrected to show her successful completion
of the Emergency Medical Training (EMT) Basic Course and the Pre-Hospital
Trauma Life Support Provider Course.

3.  The applicant provides the following documents in support of her
application:  18 January 2005 DD Form 214; Headquarters, 70th Regional
Readiness Command Orders Number (#) 05-010-00010, dated 10 January 2005;
Headquarters, I Corps and Fort Lewis Military Personnel Division (MPD)
Orders # 013-0050, dated 13 January 2005; United States Army Human
Resources Command (AHRC), St. Louis Orders # A-08-405246, dated 7 June
2004; Military Training Network Resuscitative Medicine Training Program and
Pre-Hospital Trauma Life Support Certification Cards; and Emergency Medical
Technician Basic Course, Pre-Hospital Trauma Life Support Provider Course,
Department of Defense Smallpox Vaccination Standard Training, and Trauma
Airways, Intravenous, Medication, Shock (AIMS) Course Completion
Certificates.

CONSIDERATION OF EVIDENCE:

1.  The applicant’s military records show that while serving as a member of
the United States Army Reserve (USAR), she was ordered to active duty in
support of Operation Enduring Freedom on 27 January 2003.  She served in
military occupational specialty (MOS) "91W" (Medical Noncommissioned
Officer).

2.  On 15 January 2005, the applicant was promoted to SSG/E-6.  Her
promotion was announced in Headquarters, 70th Regional Readiness Command
Orders # 05-010-00010, dated 10 January 2005.

3.  The applicant continued to serve on active duty until she was honorably
released from active duty (REFRAD) on 18 January 2005, by reason of
completion of required service.  The separation document issued to her at
that time shows she completed 1 year, 11 months, and 22 days of active duty
service during the period covered by the DD Form 214.

4.  Item 4a of the separation document contains the entry "SGT" and Item 4b
contains the entry "E5".  Item 14 contains the entry "None”, and the
applicant authenticated this document with her signature in Item 21
(Signature of Member Being Separated).

5.  The applicant’s Military Personnel Records Jacket (MPRJ) contains an
Academy of Health Sciences United States Army Diploma, which shows she
successfully completed the Medical Specialist Course, Military Occupational
Specialty (MOS) "91B1O" on 2 August 1995.  This document also shows she
attended this course from 22 May through 2 August 1995, and that it
included 130 hours of Emergency Medical Training (EMT).

6.  The applicant’s MPRJ also contains a DD Form 214 with a 30 September
1996 effective date.  Item 14 of this separation document does reflect that
the applicant completed the Medical Specialist Course in August 1995.

7.  The applicant provides certificates to show she completed the following
training courses during the period of service covered by her 18 January
2005 separation document:  Emergency Medical Technician Basic Course;
Pre-Hospital Trauma Life Support Provider Course; Department of Defense
Smallpox Vaccination Standard Training and the Trauma Airways; and
Intravenous, Medication, Shock (AIMS) Course.  The Emergency Medical
Technician Basic Course completion certificate shows she successfully
completed 168 hours of training during the period 23 October 2003 through
3 December 2003.

8.  Army Regulation 635-5 (Separation Documents) prescribes the separation
documents that are prepared for individuals upon retirement, discharge, or
release from active military service or control of the Army.  It also
establishes standardized policy for preparing and distributing the DD Form
214.  Paragraph 2-4h(4) states, in effect, that the active duty grade of
rank and pay grade at the time of separation will be entered in Item 4a and
Item 4b of the DD Form 214.

9.  Paragraph 2-4h(14) of the Separation Documents regulation states that
formal in-service (full-time attendance) training courses successfully
completed during the period of service covered by the DD Form 214 will be
listed in Item 14. This information is to assist the Soldier in job
placement and counseling; therefore, training courses for combat skills
will not be listed.

10.  Department of the Army (DA) Pamphlet (Pam) 351-4 is the official
source of information of formal courses of instruction offered at United
States Army Schools and Training Centers.
11.  DA Pam 351-4 defines a formal school course as a course operated to
support Army-wide requirements as distinguished from a course operated by a
major command for the benefit of that command and it states that all
courses that are described in DA Pam 351-4 are formal courses. Table 4-1
provides the scope for training in military occupational specialty (MOS)
"91B" (Medical Specialist).  It states, in pertinent part, that other
medical emergency situations are addressed and that the National Registry
Exam for Emergency Medical Technician certification is taken during the
course.  The Pre-Hospital Trauma Life Support Provider Course, Department
of Defense Smallpox Vaccination Standard Training, and the Trauma Airways,
Intravenous, Medication, Shock (AIMS) courses are not listed as formal
courses.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's contention that her final separation document should be
corrected to show she separated in the rank of SSG/E-6 and the military
training that she received has been carefully considered, and there is
sufficient evidence to support granting a portion of the requested relief.

2.  The evidence of record confirms that the applicant was promoted to the
rank of SSG/E-6 on 15 January 2005, just three days prior to her separation
from active duty, as announced in Headquarters, 70th Regional Readiness
Command Orders 05-010-00010, dated 10 January 2005.  Therefore the
applicant's
18 January 2005 separation document should be corrected to show that her
rank and pay grade at that time was SSG/E-6.

3.  The evidence of record also confirms that the applicant successfully
completed the Medical Specialist Course during a previous enlistment which
is accurately reflected in Item 14 on her 30 September 1996 separation
document.  She successfully completed this course a second time on 2 August
1995, eight years later, after being ordered to active duty in support of a
military operation.  This training is reflected in DA Pam 351-4 as a sub-
course taken in conjunction with Medical Specialist Training.  As a result,
Item 14 of her 18 January 2005 separation document should be corrected to
reflect "Emergency Medical Technician Basic Course, 7 weeks, December
2003."

4.  The Pre-Hospital Trauma Life Support Provider Course is not listed as a
formal course in DA Pam 351-4.  Therefore, it does not meet the regulatory
criteria necessary for inclusion on of the applicant's 18 January 2005
separation document.

5.  Although the applicant did not specifically request to have her
separation document corrected to show she completed the Department of
Defense Smallpox Vaccination Standard Training and the Trauma Airway,
Intravenous Medication, Shock (AIMS) Courses, she included Certificates of
Completion for these courses with her application.  These courses are also
not listed as formal courses in the Department of the Army Formal Schools
Catalog.  Therefore, they also do not meet the regulatory criteria
necessary for inclusion in Item 14 of the DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to
warrant a recommendation for partial relief.  As a result, the Board
recommends that all Department of the Army records of the individual
concerned be corrected by amending her 18 January 2005 DD Form 214 by
deleting the entry in Item 4a and replacing it with "SSG"; by deleting the
entry in Item 4b and replacing it with "E6"; by adding the entry "Emergency
Medical Technician Basic Course,
7 weeks, December 2003" to Item 14; and by issuing the applicant a
correction to her 18 January 2005 separation document that reflects these
changes.

2.  The Board further determined that the evidence presented is
insufficient to warrant a portion of the requested relief.  As a result,
the Board recommends denial of so much of the application that pertains to
adding any of the additional courses requested to Item 14 of her 18 January
2005 DD Form 214.
