THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that items 4a (Grade, Rate or Rank),
4b (Pay Grade), and 12h (Effective Date of Pay Grade) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending
25 August 2008 be corrected to show he was promoted to major/pay grade O4 effective 30 May 2008.
2. The applicant states that he was promoted to major effective 30 May 2008 and that he received his DD Form 214 for the period ending 25 August 2008 in late June. He points out that his promotion orders were published on 1 July 2008 and that he was on terminal leave when he received his promotion orders.
3. The applicant provides promotion orders and a copy of his DD Form 214 for the period ending 25 August 2008.
CONSIDERATION OF EVIDENCE:
1. Having prior service, the applicant was ordered to active duty on 24 April 2007 in support of Operation Enduring Freedom. He was released from active duty on 25 August 2008.
2. Item 4a on the applicants DD Form 214 for the period ending 25 August 2008 shows the entry, CPT [captain]. Item 4b on this DD Form 214 shows the entry, O03. Item 12h on this DD Form 214 shows the entry, 2002 03 01.
3. Headquarters, U.S. Army Human Resources Command, St. Louis, Missouri Orders B-07-804102, dated 1 July 2008, show the applicant was promoted to major in the U.S. Army Reserve effective 30 May 2008.
DISCUSSION AND CONCLUSIONS:
Orders show the applicant was promoted to major effective 30 May 2008. Therefore, items 4a, 4b, and 12h on his DD Form 214 for the period ending
25 August 2008 should be corrected.

b. adding the entry, MAJ [major] in item 4a on this DD Form 214;
c. adding the entry, O4 in item 4b on this DD Form 214; and
d. adding the entry, 2008 05 30 in item 12h on this DD Form 214.
