THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of items 4a (Grade, Rate or Rank), 4b (Pay Grade), and 12h (Effective Date of Pay Grade) on his DD Form 214 (Certificate of Release or Discharge from Active Duty). 2. The applicant states he was promoted to major while on terminal leave and after he signed his DD Form 214. 3. The applicant provides promotion orders in support of his application. CONSIDERATION OF EVIDENCE: 1. Having prior inactive service in the U.S. Army Reserve and the Army National Guard, the applicant was ordered to active duty on 14 May 1999 to fulfill his active Army requirement. He served as an infantry officer in Bosnia and Afghanistan. He was promoted to captain on 1 June 2001. He was honorably discharged on 31 October 2007 for miscellaneous/general reasons. 2. Item 4a on the applicant's DD Form 214 shows the entry "CPT" [Captain]. Item 4b on his DD Form 214 shows the entry "O03." Item 12h on his DD Form 214 shows the entry "2001 06 01" [1 June 2001]. 3. Promotion orders, dated 3 October 2007, show the applicant was promoted to major effective 2 October 2007 with a date of rank (DOR) of 2 October 2007. 4. Records at the Defense Finance and Accounting Service show the applicant was discharged in pay grade O-4 [major]. DISCUSSION AND CONCLUSIONS: The promotion orders which show the applicant was promoted to major on 2 October 2007 with a DOR of 2 October 2007 are accepted as sufficient evidence on which to amend items 4a, 4b, and 12h on the applicant's DD Form 214.
