THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that Item 12c (Net Service This Period) and Item 14 (Military Education) of her 22 February 2007 separation document
(DD Form 214) be corrected.   

2.  The applicant states, in effect, that she served over 8 years and this should be reflected in Item 12c of her DD Form 214.  She also states that she completed the Primary Leadership Development Course (PLDC) and this should be reflected in Item 14 of her separation document.  

3.  The applicant provides an Enlisted Records Brief (ERB) in support of her application.  

CONSIDERATION OF EVIDENCE:

1.  The applicant's record shows that she enlisted in the Regular Army and entered active duty on 10 November 1999.  It further shows she was trained in and awarded military occupational specialty (MOS) 77F (Petroleum Supply Specialist), which was later converted to MOS 92F.   

2.  The applicant's Official Military Personnel File (OMPF) contains an Academic Evaluation Report (DA Form 1059), dated 23 April 2004, which shows she attended the MOS 92F PLDC from 24 March through 23 April 2004, and that she achieved course standards and successfully completed the course.  


3.  The record confirms the applicant continuously served on active duty until 
22 February 2007, at which time she was honorably released from active duty (REFRAD), in the rank of sergeant (SGT), and transferred to the United States Army Reserve (USAR).  The DD Form 214 she was issued upon her REFRAD shows she completed 4 years of active military service in Item 12c, and the entry "None" is contained in Item 14.  Item 29 (Dates of Time Lost During This Period) contains the entry "None."  

4.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army.  It also establishes standardized policy for preparing and distributing the DD Form 214.  Chapter 2 contains item-by-item preparation instructions for the DD Form 214.  The instructions for Item 12c state, in pertinent part, that the amount of service completed, computed by subtracting Item 12a (Date Entered AD This Period) from 12b (Separation Date This Period) minus lost time and non-creditable service, will be entered.  The instructions for Item 14 state, in pertinent part, that formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214 will be entered.  

DISCUSSION AND CONCLUSIONS:

1.  The applicant's contention that Item 12c and Item 14 of her separation document should be corrected was carefully considered and found to have merit. 

2.  By regulation, the amount of service entered in Item 12c of the DD Form 214 will be arrived at by subtracting the date in Item 12a from the date in Item 12b, minus any time lost.  The evidence of record confirms that the applicant entered active duty on the period covered by the DD Form 214 on 10 November 1999, and that she continuously served until 22 February 2007.  It further confirms she accrued no time lost during the period covered by the DD Form 214.  As a result, the entry in Item 12c should reflect her completion of 7 years, 2 months, and 
28 days, which is arrived at by subtracting the date entered in Item 12a 
(10 November 1999) from the date entered in Item 12b (22 February 2007.  As a result, Item 12c of the applicant's DD Form 214 should be corrected accordingly. 

3.  By regulation, Item 14 will list all formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214.  The applicant's record contains a DA Form 1059 which confirms she completed a 4-week PLDC in MOS 92F on 23 April 2004.  As a result, it would be appropriate to add this course to Item 14 of her DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending her DD Form 214 as follows:  Item 12c - deleting the current entry and replacing it with the entry "0007  02  28";  Item 14 - by deleting the current entry and replacing it with the entry "Primary Leadership Development Course (MOS 92F) - 4 weeks - 2004"; and by providing her a correction to her separation document that reflects these changes.
