THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that item 12a of her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected. 2. The applicant states that item 12a reflects her initial time in the Army Reserve. She maintains that she has never served 6 years on active duty and argues that the 3 months listed in item 12c, net active service, represents the time she served on active duty for reclassification. 3. The applicant provides a copy of her DD Form 214, DA Form 1059 (Service School Academic Evaluation Report), Enlistment/Reenlistment Documents, Request for Conditional Release, and Leave and Earnings Statement. CONSIDERATION OF EVIDENCE: 1. The applicant's records show she enlisted in the United States Army Reserve (USAR) on 26 October 2000. 2. The applicant's DA Form 1059 shows that she completed the Intelligence Analyst Course at Fort Huachuca, Arizona, from 21 August 2006 through 14 December 2006. 3. Item 12a of the applicant’s DD Form 214, issued on 14 December 2006, shows she entered active duty on 25 October 2000. Her net active service in item 12c is listed as 3 months and 27 days. Additionally, item 14 (Military Education) of the applicant's DD Form 214, shows that she completed a 17-week Intelligence Analyst course in 2006. 4. Reenlistment/Enlistment documents show that on 23 January 2007 the applicant enlisted in the USAR as a cadet. She was subsequently appointed as a Reserve Commissioned Officer on 2 May 2009. 5. Army Regulation 635-5 (Personnel Separations) states that the DD Form 214 is a summary of a Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active duty service at the time of release from active duty, retirement, or discharge. The regulation further states, in pertinent part, that a DD Form 214 will be prepared on Reserve component Soldiers completing 90 days or more of continuous active duty for training. 6. Army Regulation 635-5 states, in pertinent part, to enter in item 12a the beginning date of the continuous period of active duty for issuance of this DD Form 214, for which a DD Form 214 was not previously issued. In item 12c enter the amount of service this period, computed by subtracting item 12a from 12b. DISCUSSION AND CONCLUSIONS: 1. It is apparent that an error occurred in the preparation of the applicant's DD Form 214 issued on 14 December 2006. The period of time between the date she entered active duty and the date she was separated does not equal 3 months and 27 days, as shown on the DD Form 214. 2. The applicant's DD Form 1059 verifies that she attended the Intelligence Analyst course from 21 August 2006 through 14 December 2006. This information is consistent with the information contained in block 14 of her DD Form 214. Therefore, item 12a should be corrected to show 21 August 2006 as the date she entered active duty. Additionally, item 12c should also be corrected to show the net active service as 3 months and 24 days.
