THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 13 June 1992 to show a date of entry of 9 June 1991.
2. The applicant states the error in the date is most likely due to changing his training from the AH-64 Apache Qualification Course to the UH-60 Qualification Course. His orders were amended due to the difference in the length of the courses. He needs the correction to show his proper length of service on active duty for retirement from the military and employment with the Department of Justice.
3. The applicant provides copies of his DD Form 214 for the period ending
13 June 1992 and two DA Forms 1059 (Service School Academic Evaluation Report).
CONSIDERATION OF EVIDENCE:
1. The applicant, a former enlisted member of the Army National Guard, was commissioned an Army National Guard officer on 13 May 1988. He served as a Field Artillery Corps officer and a Chemical Corps officer prior to transferring to the Aviation Corps in 1991.
2. Orders 162-224, issued by the U.S. Army Aviation Center, Fort Rucker, AL, dated 11 June 1991, ordered the applicant to perform aviation service and begin entitlement to Aviation Career Incentive Pay (ACIP), effective 10 June 1991.
3. His record contains a DA Form 1059 that shows he completed the:
* Initial Entry Rotary Wing Aviator (AH-1) course during the period 9 June 1991 through 24 April 1992
* UH-60 Aviator Qualification Course during the period 30 April 1992 through 11 June 1992
4. The applicant was honorably released from active duty on 13 June 1992. The DD Form 214 he was issued at the time shows in:
* item 12a (Dated Entered Active Duty This Period) the entry "91 10 18" [18 October 1991]
* item 12c (Net Active Service This Period) the entry "0000 07 26"
* item 14 (Military Education) the entry "UH-60 Aviator Qualification Course, 6 Wks, Jun 92"
5. Army Regulation 635-5 (Personnel Separations - Separation Documents), in effect at the time, prescribed the separation documents that would be furnished each individual separated from the Army and established standardized procedures for the preparation and distribution of these documents. The purpose of a separation document is to provide the individual with documentary evidence of his/her military service. It is a vital record for interested Government agencies which assist the veteran in obtaining the rights and benefits to which he/she is entitled. It states for:
a. item 12a, enter the beginning date of the continuous period of active duty for issuance of this DD Form 214, for which a DD Form 214 was not previously issued;
b. item 12c, enter the amount of service this period, computed by subtracting item 12a from 12b (Separation Date This Period); and
c. item 14, list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214. Include title, length in weeks, and year completed. This information is to assist the Soldier in job placement and counseling; therefore, do not list training courses for combat skills.
DISCUSSION AND CONCLUSIONS:
1. The evidence of record shows the applicant entered active duty on 9 June 1991 to attend the Initial Entry Rotary Wing Aviator Course and he continued on active duty to complete the UH-60 Aviator Qualification Course. He was released from active duty on 13 June 1992 at the completion of his consecutive training courses.
2. The DD Form 214 he was issued for the period ending 13 June 1992 shows he entered active duty this period on 18 October 1991; however, evidence of record shows he entered active duty on 9 June 1991 to attend the Initial Entry Rotary Wing Aviator course. Therefore, item 12a of his DD Form 214 for this period should be corrected to show he entered active duty on 9 June 1991.
3. With the above correction, it would be appropriate to correct item 12c to show he completed 1 year and 5 days of net active service this period.
4. The applicant completed the Initial Entry Rotary Wing Aviator course during this period, which was omitted from the DD Form 214. Therefore, it would be appropriate to add this course to his DD Form 214 accordingly.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that the State Army National Guard records and the Department of the Army records of the individual concerned be corrected by:
a. deleting from item 12a of his DD Form 214 for the period ending 13 June 1992 the entry "91 10 18" and replacing it with the entry "91 06 09";
b. deleting from item 12c of his DD Form 214 for the period ending 13 June 1992 the entry "00 07 26" and replacing it with the entry "01 00 05"; and
c. adding to item 14 of his DD Form 214 for the period ending 13 June 1992 the entry " Initial Entry Rotary Wing Aviator Course, 45 Wks, Apr 92."
