THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, correction of items 12a (Dated Entered AD [Active Duty] This Period) and 12c (Net Active Service This Period) of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 9 March 2006.
2. The applicant states, in effect:
* This DD Form 214 does not include her active duty service for training for military occupational specialty (MOS) 68P (Radiology Specialist) from
22 September 2004 to 15 September 2005
* Upon completion of MOS 68P training she was sent to the Warrior Transition Unit because of an injury
* There was no break in service between the two assignments at Fort Gordon
3. The applicant provides:
* Two orders for MOS 68P training
* DA Form 1059 (Service School Academic Evaluation Report)
* DD Form 214 for the period ending 9 March 2006
CONSIDERATION OF EVIDENCE:
1. Having prior enlisted service in the Air National Guard of Texas, she enlisted in the Army National Guard on 15 June 2001.
2. Orders show she was ordered to active duty for training on 22 September 2004.
3. A DA Form 1059 shows she attended the Radiology Specialist Course during the period 27 September 2004 through 15 September 2005 and she achieved course standards.
4. On 9 March 2006, the applicant was released from active duty.
5. Item 12a of the applicant's DD Form 214 for the period ending 9 March 2006 shows the entry "2005 09 11" [11 September 2005]. Item 12b (Separation Date This Period) shows the entry "2006 03 09" [9 March 2006]. Item 12c shows the entry "0000 05 29" [5 months and 29 days].
6. The applicant's retirement points detail statement shows she was ordered to active duty for training from 22 September 2004 to 9 March 2006.
7. She was appointed a second lieutenant in the U.S. Army Reserve on
21 January 2009 and promoted to first lieutenant on 20 July 2010.
8. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. In pertinent part, it states that the DD Form 214 is a synopsis of the Soldiers most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. The regulation states, in pertinent part, that for item 12 (Record of Service) use extreme care in completing this block since post-service benefits, final pay, retirement credit, and so forth are based on this information. It further states that for:
a. item 12a, enter the beginning date of the continuous period of active duty for issuance of this DD Form 214, for which a DD Form 214 was not previously issued;
b. item 12b, enter the Soldiers transition date; and
c. item 12c, enter the amount of service this period by subtracting item 12a from 12b.
DISCUSSION AND CONCLUSIONS:
The applicant's retirement points detail statement which shows she was ordered to active duty for training from 22 September 2004 through 9 March 2006, for a period of 1 year, 5 months, and 17 days, is accepted as sufficient evidence in which to correct items 12a and 12c of her DD Form 214 for the period ending
9 March 2006.
