THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show completion of the Warrior Leader Course and Unit Supply Training.

2.  The applicant states that the training courses were left off her DD Form 214 due to an administrative oversight. 

3.  The applicant provides copies of her certificates of training in support of her application. 

CONSIDERATION OF EVIDENCE:

1.  The applicant's records show that she enlisted in the Regular Army for a period of 3 years and 21 weeks on 22 September 2004.  She completed basic combat and advanced individual training and was awarded military occupational specialty (MOS) 92Y (Unit Supply Specialist).  The highest rank/grade she attained during her military service was sergeant (SGT)/E-5.

2.  The applicant's records further show that she served in Iraq from 12 November 2005 to 6 November 2006.  She was honorably released from active duty for completion of required service and transferred to the U.S. Army Reserve Control Group (Reinforcement) on 15 February 2008.

3.  Item 14 (Military Education) of her DD Form 214 shows she completed the one-week Combat Life Savers Course in 2006 and the 2-week Unit Armorer Course in 2007.  Item 14 does not show completion of the Warrior Leader Course or Unit Supply Training.

4.  The applicant's DA Form 1059 (Service School Academic Evaluation Report), dated 15 November 2007, shows the applicant completed the 4-week Warrior Leader Course at Fort Sill, Oklahoma, on 15 November 2007.

5.  Department of the Army, Certificate of Training, dated 27 April 2008, shows the applicant completed the one-week Unit Supply Training at Fort Hood, Texas, from 23 April 2007 to 27 April 2007.

6.  Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214.  The regulation directs, in pertinent part, that the purpose of the separation document is to provide the individual with documentary evidence of their military service.  It is important that information entered on the form should be complete and accurate.  Chapter 2 of Army Regulation 635-5 contains guidance on the preparation of the DD Form 214.  It states, in pertinent part, that the Soldier's military education in Item 14 is obtained from the Enlisted Record Brief (ERB).  It shows the formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214 and includes title, length in weeks, and year completed.  This information is to assist the Soldier in job placement and counseling; therefore, training courses for combat skills are not listed. 

DISCUSSION AND CONCLUSIONS:

The evidence of record shows that the applicant completed the 4-week Warrior Leader Course in 2007 and the one-week Unit Supply Training in 2007, which are not shown on her records; therefore she is entitled to correction of her records to show these courses.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the entries "Warrior Leader Course, 4 Weeks, 2007" and "Unit Supply Training, 1 Week, 2007" to Item 14 of the applicant's DD Form 214, dated 15 February 2008.
