THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show:

* she was not provided a complete dental examination and all of the appropriate dental services and treatment that she was authorized within 90 days prior to separation
* a second award of the Army Good Conduct Medal

2.  The applicant states her second award of the Army Good Conduct Medal is not shown on her DD Form 214.  Additionally, she was recently advised by a dentist at the Department of Veterans Affairs (VA) to have her DD Form 214 changed from "Yes" to "No" to indicate she was not provided a complete dental examination and all of the appropriate dental services and treatment that she was authorized within 90 days prior to separation.  She had a filing placed in her tooth during active service and now that tooth is sensitive.

3.  The applicant provides her DD Form 214, orders awarding her the Army Good Conduct Medal (2nd Award), and a VA dental summary sheet.

CONSIDERATION OF EVIDENCE:

1.  The applicant's records show she enlisted in the Regular Army (RA) on 
12 May 2004 and she held military occupational specialty 92Y (Unit Supply Specialist).  She served in Korea from 10 October 2004 to 10 September 2006 and Okinawa from 20 October 2006 to 10 November 2008.  She also executed a 4-year reenlistment on 21 April 2006.

2.  Permanent Order Number 135-04, issued by Headquarters, 55th Signal Battalion, awarded her the Army Good Conduct Medal (1st Award) for service from 13 May 2004 through 12 May 2007.

3.  She completed the Warrior Leader Course at the Noncommissioned Officer (NCO) Academy, Hawaii, from 13 November to 12 December 2007.

4.  Permanent Orders Number 133-16, issued by the Installation Management Command, Fort Belvoir, VA, on 13 May 2010, awarded her the Army Good Conduct Medal (2nd Award) for service from 12 May 2007 to 11 May 2010.

5.  She was honorably discharged on 20 August 2010.  Her DD Form 214 shows she completed 6 years, 3 months, and 9 days of creditable active service.  Additionally, this form shows in:

* Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) the:

* Army Achievement Medal (3rd Award)
* Army Superior Unit Award
* Army Good Conduct Medal
* National Defense Service Medal
* Global War on Terrorism Service Medal
* Global War on Terrorism Expeditionary Medal
* Korea Defense Service Medal
* Army Service Ribbon
* Overseas Service Ribbon (2nd Award)

* Item 17 (Member Was Provided Complete Dental Examination and All Appropriate Dental Services and Treatment within 90 days Prior to Separation) of his DD Form 214 is marked with an "X" under the "Yes" block

6.  The applicant's complete dental records are not available for review with this case; however, she submitted one document that shows she received a prophylaxis and fluoride treatment on 6 August 2010 and a restoration and a pulp cap on 16 August 2010. 

7.  It is unclear if she underwent any further dental procedures between 16 and 20 August 2010 (date of release from active duty) and it is unclear if she outprocessed the dental clinic. 

8.  Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214.  Chapter 2 of Army Regulation 635-5 contains guidance on the preparation of the DD Form 214.  It states, in pertinent part, that item 17 is applicable only to Soldiers who have completed 180 days or more of continuous active duty (check the Standard Form (SF) 603 (Health Record-Dental) for validity of exam and all appropriate treatment).  

9.  Army Regulation 600-8-22 (Military Awards) states the NCO Professional Development Ribbon.  It is awarded to Active Army, Army National Guard, and U.S. Army Reserve Soldiers for successful completion of designated NCO professional development courses.  It consists of the basic ribbon with numeral devices of 2, 3, or 4, which signify satisfactory completion of the respective levels of NCO professional development courses.

DISCUSSION AND CONCLUSIONS:

1.  The applicant was awarded two awards of the Army Good Conduct Medal, one of which is not shown on her DD Form 214; therefore, she is entitled to correction of her DD Form 214 to show both awards.  

2.  The applicant successfully completed the Warrior Leader Course, the first level of the NCO professional education system.  Therefore, she is entitled to award of the NCO Professional Development Ribbon with Numeral 1 and correction of her DD Form 214 to show it. 

3.  With respect to the dental entry, her dental records are not available for review with this case.  She submitted a document that indicates she received a prophylaxis and fluoride treatment on 6 August 2010 and a restoration and a pulp cap on 16 August 2010.  In the absence of her complete dental records, it must be presumed that the applicant received all of the dental services that she was authorized within 90 days of separation and the entry in item 17 of her DD Form 214 is correct.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

* deleting from item 13 of her DD Form 214 the Army Good Conduct Medal (1st Award)
* adding to item 13 of her DD Form 214 the Army Good Conduct Medal (2nd Award) and the NCO Professional Development Ribbon with Numeral 1

2.  The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief.  As a result, the Board recommends denial of so much of the application that pertains to item 17 (Member Was Provided Complete Dental Examination and All Appropriate Dental Services and Treatment within 90 days Prior to Separation) of her DD Form 214.
