THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show in:

* Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) the Army Commendation Medal, two awards of the Army Achievement Medal, and the Noncommissioned Officer (NCO) Professional Development Ribbon
* Item 14 (Military Education) Warrior Leader Course

2.  The applicant states:

* He attended and graduated from the Warrior Leader Course in April 2006
* The NCO Professional Development Ribbon was awarded in conjunction with completion of the Warrior Leader Course in April 2006
* During his service he was awarded one award of the Army Commendation Medal and two awards of the Army Achievement Medal

3.  The applicant provides:

* A DA Form 1059 (Service School Academic Evaluation Report) for the Warrior Leader Course
* His diploma for the Warrior Leader Course
* Permanent orders for the Army Commendation Medal


* Permanent orders for two awards of the Army Achievement Medal
* His DD Form 214 

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 5 May 2004.  He served as an air defense tactical operations center operator/maintainer and he was honorably discharged on 2 July 2008 by reason of disability, severance pay, non-combat related.

2.  Item 13 of his DD Form 214 shows, among other awards, one award of the Army Achievement Medal.  It does not show the Army Commendation Medal or the Noncommissioned Officer Professional Development Ribbon as authorized awards.  Item 14 of his DD Form 214 does not show he completed the Warrior Leader Course.

3.  Permanent Orders 339-04, issued by the 1st Battalion, 43rd Air Defense Artillery, dated 5 December 2005, awarded the applicant the Army Achievement Medal for outstanding performance during the period 12 to 19 November 2005.

4.  A DA Form 1059, dated 12 April 2006, shows the applicant achieved course standards in completing the Warrior Leader Course during the period 14 March 2006 through 12 April 2006.

5.  Permanent Orders 084-64, issued by the 1st Battalion, 43rd Air Defense Artillery, dated 25 March 2007, awarded the applicant the Army Achievement Medal for outstanding performance during the period 12 February to 10 March 2007.

6.  Permanent Orders Number 219-03, issued by Headquarters, 11th Air Defense Artillery Brigade, dated 6 August 2008, awarded the applicant the Army Commendation Medal for meritorious service during the period 30 January through 2 July 2008.

7.  Army Regulation 600-8-22 (Military Awards) states the NCO Professional Development Ribbon was established by the Secretary of the Army on 10 April 1981.  It is awarded to Active Army, Army National Guard, and U.S. Army Reserve Soldiers for successful completion of designated NCO professional development courses.  Paragraph 5-6e of the regulation states once a service member has been awarded the NCO Professional Development Ribbon upon graduation from WLC or Warrior Leader Course - Reserve Component, 


subsequent appropriate numerals will be awarded to identify completion of higher level NCO Education System (NCOES) or Reserve Component - NCOES courses.

8.  Army Regulation 635-5 (Personnel Separations - Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214.  It states that for item 14, list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214.  Include title, length in weeks, and year completed.  This information is to assist the Soldier in job placement and counseling; therefore, do not list training courses for combat skills.

DISCUSSION AND CONCLUSIONS:

1.  Orders show the applicant was awarded the Army Commendation Medal.  Therefore, his DD Form 214 should be corrected to show this award.

2.  His DD Form 214 shows one award of the Army Achievement Medal.  However, orders show he was awarded two awards of the Army Achievement Medal.  Therefore, his DD Form 214 should be corrected to show two awards of the Army Achievement Medal.

3.  Evidence shows the applicant completed the 4-week Warrior Leader Course on 12 April 2006.  Therefore, his DD Form 214 should be corrected accordingly.  

4.  Based on his successful completion of the Warrior Leader Course, he is entitled to the NCO Professional Development Ribbon and correction of his DD Form 214 to show this award.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting from item 13 of his DD Form 214 the Army Achievement Medal;

	b.  adding to item 13 of his DD Form 214 the:

* Army Commendation Medal
* Army Achievement Medal (2d Award)
* Noncommissioned Officer Professional Development Ribbon

	c.  adding to item 14 of his DD Form 214 the entry "Warrior Leader Course,
4 Weeks, 2006."
