	The Board considered the following evidence: 

	Exhibit A - Application for correction of military records.

	Exhibit B - Military Personnel Records (including advisory opinion, if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that his service in Qatar and Kuwait be added to his DD Form 214 (Certificate of Release or Discharge from Active Duty).

2.  The applicant states, in effect, his service in Qatar and Kuwait from  
27 October 2006 to 22 December 2006 was not included on his DD Form 214.

3.  The applicant provides a copy of his DD Form 214, an Operation Enduring Freedom Certificate of Wartime Service, an Operation Forward Presence Certificate of Achievement, and his Chronological Record of Medical Care.

CONSIDERATION OF EVIDENCE:

1.  The applicant's military records show that he enlisted in the Regular Army on 21 April 2005.  He completed basic combat training and advanced individual training and was awarded military occupational specialty (MOS) 25U1O (Signal Support System Specialist).

2.  Section IX (Assignment Information) of the applicant's Enlisted Record Brief (ERB) shows the applicant was assigned to the following units: 11th Air Defense Artillery Brigade; Headquarters and Headquarters Battery Patriot, 3rd Battalion, 43rd Air Defense Artillery; Battery E Patriot, 3rd Battalion, 43rd Air Defense Artillery; and Detachment 1, 3rd Battalion, 43rd Air Defense Artillery Fort Bliss, Texas during the period from 27 April 2006 to 21 December 2006.

3.  On 20 March 2007, the applicant was discharged due to personality disorder. He had completed 1 year and 11 days of active service that was characterized as honorable.

4.  Item 12f (Foreign Service) of the applicant's DD Form 214 does not show the applicant had any foreign service.

5.  Item 18 (Remarks) of the applicant's DD Form 214 does not contain an entry indicating any country he was deployed to and the inclusive dates.

6.  The Certificate of Wartime Service and Certificate of Achievement submitted by the applicant does not contain any inclusive dates of the applicant's deployment.  Also, the period of foreign service that the applicant is requesting to correct (27 October 2006 to 22 December 2006) does not coincide with the date on the Certificate of Wartime Service (18 December 2006).

7.  The copy of the chronological record of medical care submitted by the applicant shows a history of treatment at the Qatar Troop Medical Clinic during the period 7-12 December 2006. 

8.  In a telephone conversation between a staff member and a unit representative from the 3rd Battalion, 43rd Air Defense Artillery, Fort Bliss, Texas confirms that the unit was deployed to Qatar and Kuwait during the period 27 October 2006 to  
22 December 2006.

9.  Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214.  In pertinent part, it directs that foreign service, taken from the ERB/ORB (Enlisted Record Brief/Officer Record Brief) performed during the period covered by the DD Form 214 will be entered in item 12f.

10.  Army Regulation 635-5, in pertinent part, directs that when an active duty Soldier is deployed with his or her unit during the period covered by the DD Form 214, an entry will be made in Item 18 to show the name of the country deployed to and the inclusive dates in YYYY/MM/DD format.

DISCUSSION AND CONCLUSIONS:

1.  The applicant contends that his DD Form 214 should be corrected to show his foreign service in Qatar and Kuwait for the period 27 October 2006 to  
22 December 2006.

2.  The evidence that the applicant submitted (Certificate of Wartime Service, Certificate of Achievement, and chronological record of medical care) does not show the exact dates of the applicant's deployment to Qatar and Kuwait.  However, the dates were confirmed by a telephone conversation between a staff member and a unit representative from the 3rd Battalion, 43rd Air Defense Artillery.  Therefore, the applicant is entitled to correction of his records to show in item 12f (Foreign Service) 0000 01 26 and in item 18 (Remarks) of the  
DD Form 214 Qatar and Kuwait 20061027 – 20061222.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by showing in item 12f (Foreign Service) the entry 0000 01 26 and in item  
18 (Remarks) of her DD Form 214 Qatar and Kuwait 20061027 – 20061222.
