      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that her records be corrected to include her
third award of the Meritorious Service Medal, the Bronze Star Medal, and
7th award of the Good Conduct Medal.

2.  The applicant states that the three awards were omitted from her
retirement DD Form 214 (Certificate of Release or Discharge from Active
Duty).

3.  The applicant provides copies of her DD Form 214, three DA Forms 638
(Recommendation for Award) for the Meritorious Service Medal with 2nd Oak
Leaf Cluster, the Bronze Star Medal, and the Good Conduct Medal (7th
award).

CONSIDERATION OF EVIDENCE:

1.  The records show the applicant entered active duty on 11 January 1994
and served until she retired on 21 January 2005.

2.  The record contains copies of orders awarding the applicant the
Meritorious Service Medal on 4 November 1998 and 12 May 2000.

3.  On 15 November 2003 the Commander, 230th Finance Battalion recommended
the applicant for the award of the Bronze Star Medal.  The recommendation
was approved.

4.  Headquarters, 4th Infantry Division Permanent Orders Number 341-68
awarded her the Bronze Star Medal effective 26 December 2003.

5.  On 22 November 2004 the Commander, 230th Finance Battalion recommended
the applicant for the award of the Meritorious Service Medal.  The
applicant is shown to have already received the Meritorious Service Medal
with one Oak Leaf Cluster.  The recommendation was approved.

6.  Headquarters, III Corps and Fort Hood Permanent Orders Number A-349-8
awarded her the Meritorious Service Medal effective 14 December 2004.  The
orders list the award as the Meritorious Service Medal with 1st Oak Leaf
Cluster.

7.  230th Finance Battalion Permanent Orders 025-01, dated 25 January 2005
awarded the applicant the Good Conduct Medal (7th award).


8.  The 31 January 2005 DD Form 214 lists the applicant's awards as the
Meritorious Service Medal (2nd award), the Army Commendation Medal (4th
award), the Joint Service Achievement Medal, the Army Achievement Medal
(8th award), the Global War on Terrorism Expeditionary Medal, the Global
War on Terrorism Service Medal, the Good Conduct Medal (6th award), the
National Defense Service Medal (2nd award), the Korea Defense Service
Medal, the Noncommissioned Officers Professional Development Ribbon with
Numeral 3, the Overseas Service Ribbon (4th award), and the Marksman
Marksmanship Qualification Badge with Rifle Bar.

9.  Army Regulation 600-8-2 (Military Awards) states that an Oak Leaf
Cluster will attached to medals to denote a second or subsequent award of
the Medal.

DISCUSSION AND CONCLUSIONS:

1.  The applicant was awarded a third Meritorious Service Medal on
26 December 2003; however, the orders incorrectly list the award as the
Meritorious Service Medal with 1st Oak Leaf Cluster.  It is appropriate to
correct the records to show the applicant was awarded the Meritorious
Service Medal with two Oak Leaf Clusters (3rd award).

2.  The applicant was awarded the Bronze Star Medal in 2003 and a seventh
Good Conduct Medal on 25 January 2005.  These awards were omitted from her
DD Form 214 and it is appropriate to correct the omissions at this time.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
showing the applicant was awarded the Meritorious Service Medal with two
Oak Leaf Clusters, the Bronze Star Medal, and a seventh Good Conduct Medal.
