THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that the already-awarded Bronze Star Medal and the Army Commendation Medal be added to her DD Form 214, Certificate of Release or Discharge from Active Duty. 

2.  The applicant states, in effect, that she received these awards after her 
DD Form 214 had been completed for her release from active duty.

3.  In support of her request, the applicant provided a copy of an award certificate for award of the Bronze Star Medal and the Army Commendation Medal to her.

CONSIDERATION OF EVIDENCE:

1.  The evidence of record show the applicant entered active duty on 4 November 2000 and was honorably released from active duty in the rank of Captain on 1 June 2006. 

2.  Item 12.f. (Foreign Service), of the applicant's DD Form 214, shows she served in Kosovo for the period 28 October 2002 through 19 July 2003 and in Iraq for the period from 9 February 2004 through 28 February 2005.  The evidence shows she had service in Korea; however, the period of her service there is not available.

3.  The DD Form 214 the applicant was issued on her release from active duty shows in Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized), she was awarded:  the Army Commendation Medal, the Army Achievement Medal, the National Defense Service Medal, the Korean Service Medal, the Global War on Terrorism Expeditionary Medal, the Global War on Terrorism Service Medal, the Korean Defense Service Medal, the Kosovo Campaign Medal, the Army Service Ribbon, the Overseas Service Ribbon (3rd Award), and the North Atlantic Treaty Organization Medal [NATO Medal].  No other awards are shown.

4.  The applicant was awarded the Army Commendation Medal, for meritorious achievement, for the period 30 October 2002 through 28 July 2003, by Permanent Orders 135-5, prepared by Headquarters, Multinational Brigade, on 15 May 2003.  This award certificate is already on file in the applicant's official military personnel file and is believed to be the Army Commendation Medal shown on the applicant's DD Form 214.

5.  The applicant was awarded a second Army Commendation Medal, in effect, the Army Commendation Medal, with oak leaf cluster, for meritorious service for the period 21 March 2002 through 16 May 2005, by Permanent Orders 209-3, prepared by Headquarters, 1st Infantry Division Support Command, dated 28 July 2005.  This award is not shown on the applicant's DD Form 214.

6.  The applicant was awarded the Bronze Star Medal, for exceptionally meritorious service, for the period 11 February 2004 through 10 February 2005, by Permanent Orders 364-[remainder of number administratively omitted], prepared by Headquarters, 1st Infantry Division.  This award is not shown on the applicant's DD Form 214; however, the award certificate is on file in the applicant's official military personnel file.

7.  Item 13, of the applicant's DD Form 214, incorrectly shows that she was awarded the Korean Service Medal.  Army Regulation 600-8-22 provides for award of the Korean Service Medal to Soldiers who performed qualifying service in the theater of operations between 27 June 1950 and 27 July 1954.

8.  A bronze oak leaf cluster is issued to denote the award of a second and succeeding award of a decoration which includes the Army Commendation Medal.

9.  Army Regulation 600-8-22 prescribes the separation documents that must be prepared for soldiers on retirement, discharge, release from active duty service, or control of the Active Army. It establishes standardized policy for preparing and distributing the DD Form 214.  Instructions for the completion of Item 13 are to list all awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22.  Each entry will be verified by the soldier's records and abbreviations will not be used.

DISCUSSION AND CONCLUSIONS:

1.  The evidence shows the applicant was awarded the Bronze Star Medal and a second Army Commendation Medal, in effect, the Army Commendation Medal, with oak leaf cluster.

2.  The applicant's DD Form 214 does not show the Bronze Star Medal.  The award certificate for the Bronze Star Medal is on file in her official military personnel file.  It is believed the Bronze Star Medal was omitted from the DD Form 214 when she 

was released from active duty.  The applicant is therefore entitled to addition of the Bronze Star Medal to her DD Form 214.

3.  The applicant's DD Form 214 shows she was awarded an Army Commendation Medal.  The applicant submitted an award certificate showing she was awarded a second Army Commendation Medal, in effect, the Army Commendation Medal, with oak leaf cluster.  It is believed this second awarded Army Commendation Medal was omitted from the DD Form 214 she was issued when she was released from active duty.  The applicant is therefore entitled to addition of the second awarded Army Commendation Medal, in effect, the Army Commendation Medal, with oak leaf cluster, to her DD Form 214.

4.  The evidence shows a Korean Service Medal was erroneously added to the applicant's DD Form 214 at the time she was released from active duty.  The applicant does not meet the criteria for this service award.  To have earned this service award, the applicant would have had to serve in the Korean War between the dates of 27 June 1950 and 27 July 1954.  However, it is the policy of this Board to not disadvantage an applicant outside of their stated issue and as such will not delete this award.  The applicant may request a deletion of this award at a later date.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the already-awarded Bronze Star Medal and the Army Commendation Medal, with oak leaf cluster, to the applicant's DD Form 214.
