THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that a duplicate award of Meritorious Service
Medal (MSM) be removed from her Official Military Personnel File (OMPF).
2. The applicant states that she is in the process of updating her OMPF
for the Master Sergeant Promotion Board and discovered that an award of the
MSM for the period 1 August 2001 to 4 June 2004 is listed twice.
3. The applicant provides a copy of an award for the MSM in support of her
case.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 29 October 1985 and has
served continuously on active duty since that time. She is currently
serving as a Sergeant First Class at Fort Huachuca, Arizona.
2. The applicant’s OMPF shows she was awarded the MSM for the period
29 December 1989 to 30 May 1994 while assigned to the United States Army
Combined Arms Command at Fort Leavenworth, Kansas.
3. The applicant’s OMPF shows she was awarded an additional award of
the MSM for the period 1 July 1995 to 13 July 1998 while assigned to
Fort Leavenworth Kansas.
4. The applicant was also awarded the MSM for the period 1 August 2001 to
4 June 2004 while assigned to 3rd United States Army, Fort McPherson,
Georgia.
5. A review of the Commendatory Data portion of the applicant’s OMPF
microfiche shows duplicate award certificates of the MSM for the period 1
August 2001 to 4 June 2004.
6. Army Regulation 600-8-22 (Military Awards) provides, in pertinent part,
that the Meritorious Service Medal is awarded to members of the Armed
Forces of the United States or of a friendly foreign nation who distinguish
themselves by outstanding meritorious achievement or service in a noncombat
area. As with all personal decorations, formal recommendations, approval
through the chain of command, and announcement in orders are required.
7. Army Regulation 600-8-104 (Military Personnel Information Management/
Records) sets forth the policies, operating tasks and steps for governing
the OMPF; the Military Personnel Records Jacket; the Career Management
Individual File; Army Personnel Qualification Records; and Military
Personnel Information Management as a work category. Paragraph 2-4 of this
regulation states that once a document is placed in the OMPF it becomes a
permanent part of that file and will not be removed from that file or moved
to another part of the file unless directed by the ABCMR; the Department of
the Army Suitability Evaluation Board; Army Appeals Board; Chief of Appeals
and Corrections
Branch of the Total Human Resources Command-HRC); the OMPF custodian when
documents have been improperly filed, HRC (formerly Total Army Personnel
Command (TAPC-PDO-PO) as an exception; Chief of the Appeals Branch of the
Army Reserve Personnel Center; and Chief of the Appeals Branch of the
National Guard Personnel Center.
DISCUSSION AND CONCLUSIONS:
1. The applicant contends that a duplicate award of Meritorious Service
Medal (MSM) should be removed from her OMPF.
2. A review of the applicant’s OMPF microfiche revealed duplicate awards
of the MSM for the period 1 August 2001 to 4 June 2004. It appears the
duplicate award certificates were placed on her OMPF microfiche in error.
Therefore, one of the award certificates for the MSM for period 1 August
2001 to 4 June 2004 should be removed from her records.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that one of
the duplicate award certificates for the MSM for the period 1 August 2001
to 4 June 2004 be removed from the applicant’s OMPF.
