         The applicant and counsel if any, did not appear before the Board.

         The Board considered the following evidence:

         Exhibit A - Application for correction of military
records
         Exhibit B - Military Personnel Records (including
         advisory opinion, if any)

FINDINGS :

1. The applicant has exhausted or the Board has waived the requirement for exhaustion of all administrative remedies afforded by existing law or regulations.


2. The applicant requests that his Certificate of Release or Discharge from Active Duty, DD Form 214, be amended to add the Army Commendation Medal (ARCOM) and three awards of the Army Achievement Medal (AAM). He states that his personnel office failed to annotate his records in a timely manner and failed to maintain any record of the award orders in his files.

3. The applicant’s military records show that he enlisted in the Regular Army on 21 February 1996. He was awarded the ARCOM on Permanent Orders (PO) 320-332, 500th Military Intelligence Group dated 3 January 2001. He was awarded the AAM (the certificate does not reflect the PO number) for achievement from 22 July to 5 August 1999. He was awarded the AAM on PO 257-2, Headquarters, 8th Personnel Command dated 14 September 1999 for achievement from 18 August to 16 September 1999. He was awarded the AAM (the certificate does not reflect the PO number) for achievement from 9 February to 26 September 2000. All four of these awards are filed on his Official Military Personnel File (OMPF).

4. The applicant was released from active duty on 20 February 2001. None of the requested awards are shown on his DD Form 214.

CONCLUSIONS:

1. The evidence of record shows that the applicant was awarded the ARCOM and three awards of the AAM and they should be added to his DD Form 214.

2. In view of the foregoing, the applicant’s records should be corrected as recommended below.

RECOMMENDATION:

That the applicant’s DD Form 214 be amended to add the Army Commendation Medal and three awards of the Army Achievement Medal.
