      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that Item 13 (Decorations, Medals,
Badges, Citations and Campaign Ribbons Awarded or Authorized) and Item 14
(Military Education) of his separation document (DD Form 214) be corrected.

2.  The applicant states, in effect, that his latest separation document,
effective 23 April 2003, is missing one of his awards of the Army
Achievement Medal (AAM).  He also indicates that it fails to show he
completed the Professional Leadership Development Course and his military
occupational specialty (MOS) training.

3.  The applicant provides the following documents in support of his
application:  Course Completion Certificates; AAM Certificates; and 27 June
1999 and
23 April 2003 DD Forms 214.

CONSIDERATION OF EVIDENCE:

1.  The applicant’s military records show he initially enlisted in the
Regular Army and entered active duty on 28 June 1996.  He was trained in,
awarded, and served in MOS 14J (Early Warning System Operator).  He
continued to serve until he was honorably released from active duty
(REFRAD) on 27 June 1999, by reason of completion of required service.

2.  The separation document that he was issued on 27 June 1999 shows he
held the rank of specialist (SPC) and had completed a total of 3 years
active military service.  The awards entered in Item 13 includes the AAM
(2nd Award).  Item 14 contains the entry "None."

3.  On 24 July 2002, while serving in the Army National Guard (ARNG), the
applicant was ordered to active duty in support of contingency Operation
Joint Forge.  He served on active duty for nine months until being
honorably REFRAD on 23 April 2003.  The list of awards entered in Item 13
of the DD Form 214 he was issued for this period of active duty service
includes the AAM (3rd Award), and Item 14 contains the entry "None."

4.  The applicant provides four AAM award certificates, which confirm these
awards were announced in the following Permanent Orders (PO) on the dates
indicated:  PO Number (#) 1441-021 on 30 May 1997; PO# 190-001 on 9 July
1998; PO# 137-001 on 17 May 1999; and PO# 6-14 on 30 June 2001.
5.  The applicant provides a course completion certificate that shows he
completed 352 hours of training in Early Warning System Operator Course
Number 21-96 on 24 January 1997.  He also provides a certificate showing he
successfully completed the Reserve Component  (RC) Noncommissioned Officers
(NCO) Primary Leadership Development Course (PLDC) in March 2001.

6.  Army Regulation 635-5 prescribes the separation documents that are
prepared for individuals upon retirement, discharge, or release from active
military service or control of the Army.  It also establishes standardized
policy for preparing and distributing the DD Form 214.  Paragraph 2-4h(14)
states that formal in-service (full-time attendance) training courses
successfully completed during the period of service covered by the DD Form
214 will be listed in Item 14. This information is to assist the Soldier in
job placement and counseling; therefore, training courses for combat skills
will not be listed.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's contention that Items 13 and 14 of his 23 April 2003
separation document should be corrected to include the AAM (4th Award) and
the courses in question has been carefully considered.  There is sufficient
evidence to support granting a portion of the requested relief.

2.  The evidence of record confirms the applicant was awarded the AAM on
four separate occasions, and that the awards were properly announced and
authorized in permanent orders.  He was awarded the AAM three times prior
to his initial REFRAD on 27 June 1999, and four times prior to his final
REFRAD on 23 April 2003.  Therefore, it would appropriate to add the AAM
(3rd Award) to the list of awards contained in Item 13 of his 27 June 1999
DD Form 214, and the AAM (4th Award) to the list of awards contained in
Item 13 of his 23 April 2003 DD Form 214.

3.  By regulation, Item 14 of the DD Form 214 should document formal in-
service (full-time attendance) training courses successfully completed
during the period of service covered by the DD Form 214.  The evidence in
this case confirms the applicant completed the Early Warning System
Operator Course # 21-96 on
24 January 1997; however, this course is not reflected on his 27 June 1999
separation document.  Therefore, it would be appropriate to add this course
to Item 14 of his 27 June 1999 separation document at this time.

4.  The evidence in this case also confirms the applicant completed the RC
NCO PLDC on 24 March 2001.  This military training course was completed
after the applicant's initial period of honorable active duty service and
prior to his final period service of active duty service.  Although the
applicant did complete this course, it was not completed during the periods
of service covered by the two separation documents in question.  The
governing regulation only allows for military education completed during
the period covered by the DD Form 214 to be entered on that document.
Therefore, it would not be appropriate to add this course to Item 14 of
either of his separation documents.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to
warrant a recommendation for partial relief.  As a result, the Board
recommends that all Department of the Army records of the individual
concerned be corrected by:

      a.  amending his 27 June 1999 DD Form 214 by deleting the current Army
Achievement Medal entry in Item 13 and replacing it with the entry “Army
Achievement Medal (3rd Award)” and by adding the entry “Early Warning
System Operator Course, 9 weeks, 1997” to Item 14.


      b.  amending his 24 April 2003, DD Form 214 by deleting the current
Army Achievement Medal entry in Item 13 and replacing it with the entry
“Army Achievement Medal (4th Award)”; and


      c.  issuing him corrections to his 27 June 1999 and 23 April 2003
separation documents that reflect these changes.

2.  The Board further determined that the evidence presented is
insufficient to warrant a portion of the requested relief.  As a result,
the Board recommends denial of so much of the application that pertains to
correcting his 23 April 2003 separation document to show that he completed
the Reserve Components Noncommissioned Officers Primary Leadership
Development Course.
