APPLICANT REQUESTS THAT:
His records be corrected to reflect he declined Servicemembers’
Group Life Insurance (SGLI) in May 2006 and any debt incurred be
remitted.

APPLICANT CONTENDS THAT:
He unintentionally applied for SGLI in October 2005. He
corrected the matter in May 2006 by completing a new SGLV 8286
Servicemembers’ Group Life Insurance Election and Certificate,
stating “I do not want insurance at this time.” The Military
Personnel Flight did not upload the new form. In October 2011,
his Leave and Earnings Statement (LES) reflected the SGLI
premium had restarted and established a debt of $1989.00. The
form he completed in May 2006 was burned in an apartment fire.
The only record he has to prove his updated election is his May
2006 LES showing a refund of the SGLI premiums.
In support of his appeal, the applicant provides SGLV 8286,
copies of his LES, and DD Form 2789, Waiver/Remission of
Indebtness Application.

STATEMENT OF FACTS:
The applicant is on active duty serving in the grade of staff
sergeant.
The remaining relevant facts pertaining to this application are
contained in the letter prepared by the Air Force office of
primary responsibility which can be found at Exhibit C.

AIR FORCE EVALUATION:
AFPC/DPWC recommends approval of remitting the debt. The
applicant was under full SGLI since October 2005 and monthly
premiums were deducted from his pay. In April 2006, records
show a computer transaction from the Military Personnel Data
System (MILPDS) arrived into the finance system and stopped the
SGLI premiums effective October 2005. This transaction caused
the finance system to refund the premiums collected during that
time. In October 2011, a transaction was sent by MILPDS to the
finance system to restart the SGLI premiums effective October
2005, which created the debt.
DPWC opines the computer transaction in April 2006 was the
result of the applicant attempting to decline SGLI. However,
the declination was never placed into his official record. His
current duty station reviewed his records and found the October
2005 SGLI election. Accordingly, it updated MILPDS to reflect
that election thus creating the debt. The applicant challenged
the change; however, he could not produce the SGLV 8286 from
April 2006 declining coverage.
The computer transaction in April 2006 shows the applicant’s
intent to stop the SGLI coverage. He should be relieved of the
debt incurred.

APPLICANT'S REVIEW OF THE AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the
applicant on 11 May 2012, for review and comment within 30 days.
As of this date, no response has been received by this office
(Exhibit D).

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by law or
regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to demonstrate
the existence of an error or injustice to warrant relief. After
thoroughly reviewing the evidence submitted in support of his
appeal, we agree with the Air Force office of primary
responsibility assessment that sufficient evidence has been
provided to show the applicant’s intent to stop his SGLI. It
appears that due to administrative errors beyond the applicant’s
control, reinstatement of full SGLI coverage was erroneously

completed. Accordingly, we recommend his records be corrected as
indicated below.

THE BOARD DETERMINES THAT:
The pertinent military records of the Department of the Air
Force relating to APPLICANT, be corrected to show that he
executed SGLV 8286, Servicemembers’ Group Life Insurance
Election and Certificate, declining any and all coverage,
effective 24 October 2005.
