APPLICANT REQUESTS THAT:
Her DD Form 214, Certificate of Release or Discharge from Active
Duty, be corrected to reflect:
1. Her last name of be corrected.
2. Her pay grade be changed from E-4 (senior airman) to E-5
(staff sergeant).
3. Her Foreign Service be changed from 0 to 2 years
(Partially administratively corrected by AFPC to reflect 1 year,
11 months, and 27 days).
4. Her separation code and reentry (RE) code be added to the
DD Form 214.

APPLICANT CONTENDS THAT:
Her DD Form 214 is incorrect and has missing information.
She was an E-5, with a date of rank of 1 August 2009 before she
was honorably discharged; however, her DD Form 214 Worksheet
reflects E-4.
She had Foreign Service at Kadena Air Base, Okinawa, Japan from
2006 through 2008; however, no Foreign Service is reflected on
her DD Form 214 Worksheet.
She was married in December 2008 and her last name was changed
to XXXX; however, the change is not listed on her DD Form 214.
In support of the request, the applicant provides copies of her
DD Form 214 Worksheet, personnel data from the Military
Personnel Data System (MilPDS), and marriage certificate.

STATEMENT OF FACTS:
Available records reflect the applicant entered the Regular Air
Force on 27 September 2005.
She enlisted as an airman basic and was progressively promoted
to the grade of staff sergeant, with an effective date of
promotion of 1 August 2009.
The applicant was honorably discharged on 26 September 2009.
Her DD Form 214 reflects her narrative reason for separation as
completion of required service, her separation code as MBK, and
reenty code as 1J (eligible to reenlist, but elects separation).
In accordance with AFI 36-3202, Separation Documents, the
DD Form 214 Worksheet is a document used to help personnel
prepare the actual DD Form 214, Certificate of Release or
Discharge from Active Duty. The DD Form 214 Worksheet is
completed by using the unit personnel records group (UPRG)
information contained in the report on individual personnel
(RIP) from the Air Force Personnel Center. The separating or
retiring individual is required to review the worksheet prior to
the completion of the DD Form 214.
HQ AFPC/DPAPP verified and confirmed the applicant had boots on
ground Foreign Service at Kadena Air Base, Japan, from 21 April
2006 to 16 April 2008 and have indicated the records will be
administratively corrected to reflect this.

AIR FORCE EVALUATION:
HQ AFPC/DPSIRP recommends denial with regard to changing the
name on her DD Form 214. DPSIRP states the applicant was
advised on 26 May 2010 that either the original or a certified
or a notarized copy of her marriage certificate was needed and,
upon receipt, her name would be changed in her service record.
As of this date, the required documentation has not been
provided.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the
applicant on 16 July 2010 for review and comment within 30 days
(Exhibit D). As of this date, this office has not received a
response.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was timely filed.
3. Insufficient relevant evidence has been presented to
demonstrate the existence of error or injustice with regard to
the applicants name change, reentry code, and Foreign Service
request. In this respect, we note the applicant has been
advised of the required documentation to effect a name change;
however, she has failed to provide the requested documentation.
Further, we note that her reentry code is properly reflected on
her DD Form 214 and that her Foreign Service has been verified
and will be administratively corrected on her DD Form 214.
Therefore, in view of the above and absent any evidence to the
contrary, we find no basis to recommend granting this portion of
the application.
4. Sufficient relevant evidence has been presented to
demonstrate the existence of error or injustice with regard to
the applicants rank and grade on her DD Form 214. In this
respect, we note the applicant was promoted to the grade of
staff sergeant with a date of rank of 1 August 2009 prior to her
discharge. Therefore, we recommend her records be corrected to
the extent indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air
Force relating to APPLICANT, be corrected to show that on
26 September 2009, she was discharged in the pay grade of E5,
staff sergeant.
