APPLICANT REQUESTS THAT:
Her DD Form 214, Certificate of Release or Discharge from Active
Duty, be amended with a DD Form 215, Correction to DD Form 214,
Certificate of Release or Discharge from Active Duty, to reflect
her name as Vanessa J. Peay.

APPLICANT CONTENDS THAT:
Her record does not contain an error. She is requesting the
record be amended to reflect her maiden name, not married name.
The applicant did not provide any supporting documentation.
The applicants complete submission is at Exhibit A.

STATEMENT OF FACTS:
The applicant is a former member of the Regular Air Force who
served from 16 March 2004 to 16 March 2010. She was trained and
served as a Munitions Systems Journeyman and was progressively
promoted to the rank of Staff Sergeant (SSgt) with a pay grade
of E-5.
The applicant was discharged with an honorable character of
service and credited with 6 years of active duty service which
includes 2 years, 8 months and 2 days of Foreign and/or Sea
Service.
The applicants DD Form 214, reflects the name Vanessa Jannette
Overturff.
By letter dated 26 September 2011, AFPC/DPSSRP advised the
applicant that they required a source document to be able to
change her name in the personnel records. They requested either
the original document or a certified/notarized copy.
Additionally they advised the applicant that since her name
changed after her release from active duty, her DD Form 214
would not be amended.

AIR FORCE EVALUATION:
AFPC/DPSIRP recommends denial. DPSIRP states, the applicant did
not provide supporting documentation with her request. Air
Force Instruction (AFI) 36-2608, Military Personnel Records
Systems, Table A7, makes clear the documentation required to
change or correct a name is a birth, baptismal, marriage, or
naturalization certificate, civil court order, or other legal
document. As the applicant has not provided the required
document, they recommend her request be denied.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the
applicant on 12 March 2012 (Exhibit D) for review and comment
within 30 days. As of this date, this office has received no
response.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Insufficient relevant evidence has been presented to
demonstrate the existence of error or injustice. We took notice
of the applicant's complete submission in judging the merits of
the case; however, we agree with the opinion and recommendation
of the Air Force office of primary responsibility and adopt its
rationale as the basis for our conclusion that the applicant has
not been the victim of an error or injustice. Therefore, based
on the available evidence of record, we find no basis upon which
to favorably consider this application.

THE BOARD DETERMINES THAT:
The applicant be notified that the evidence presented did not
demonstrate the existence of material error or injustice; that
the application was denied without a personal appearance; and
that the application will only be reconsidered upon the
submission of newly discovered relevant evidence not considered
with this application.

Her records be corrected to reflect her name change. For these reasons, she believes her request to change her gender on her military records be allowed. We note the SAF/MR memorandum dated 9 March 2015, states that corrections to the DD Form 214 should be for the limited purposes of mitigating an injustice caused by use of the DD Form 214.
