APPLICANT REQUESTS THAT:

His DD Form 214, Separation or Release from Active Duty, be corrected to reflect that he was awarded the Air Force Achievement Medal (AFAM).

APPLICANT CONTENDS THAT:

He was awarded the AFAM, but did not receive it until after his discharge.

In support of his request, the applicant provides copies of his DD Form 214 and AF Form 2274, Air Force Achievement Medal Certificate.

Applicant’s complete submission, with attachments, is at Exhibit A.

STATEMENT OF FACTS:

Applicant’s military records indicate he enlisted in the Regular Air Force as an airman basic (E-1) on 23 Jun 03 for a period of four years. He was progressively promoted to the rank of senior airman (E-4) with an effective date and date of rank of 25 Jun 06.

On 23 Jun 07, the applicant was voluntarily released from active duty at the expiration of his term of service with service characterized as honorable. He was credited with four years of active service. His DD Form 214 reflects that he was entitled to wear the National Defense Service Medal, Global War on Terrorism Service Medal, and Air Force Training Ribbon at the time of his release.

The AFAM is awarded for outstanding achievement or meritorious service and may also be awarded for acts of courage lesser than for award of the Air Force Commendation Medal.

AIR FORCE EVALUATION:

AFPC/DPSIDR recommends denial, indicating the decoration is invalid without special orders in accordance with AFI 36-2803, The Air Force Awards and Decorations Program. Specifically, the AFI states, “Once an award has been approved, prepare a certificate (attachment 7), impress a seal on the citation and publish special orders.”

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

A copy of the Air Force evaluation was forwarded to applicant on 23 Dec 08 for review and comment within 30 days. As of this date, no response has been received by this office (Exhibit D).

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by existing law or regulations.

2.  The application was timely filed.

3.  Sufficient relevant evidence has been presented to demonstrate the existence of probable error or injustice warranting corrective action. We note the AFAM citation provided by the applicant in support of his appeal. It reflects the applicant was awarded the AFAM for meritorious service during the period 8 Jan 04 to 23 Jun 07. Having no basis to question the validity of this citation, we recommend that the applicant’s records be corrected to the extent indicated below.

THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air Force relating to APPLICANT be corrected to show that his DD Form 214, Certificate of Release or Discharge from Active Duty, issued in conjunction with his release from active duty on 23 Jun 07, be amended in Item 13, Decoration, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized, to reflect award of the Air Force Achievement Medal.

MEMORANDUM FOR THE CHIEF OF STAFF

Having received and considered the recommendation of the Air Force Board for Correction of Military Records and under the authority of Section 1552, Title 10, United States Code (70A Stat 116), it is directed that:

The pertinent military records of the Department of the Air Force relating to APPLICANT, XXX-XX-XXXX, be corrected to show that his DD Form 214, Certificate of Release or Discharge from Active Duty, issued in conjunction with his release from active duty on 23 Jun 2007, be, and hereby is, amended in Item 13, Decoration, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized, to reflect award of the Air Force Achievement Medal.
