APPLICANT REQUESTS THAT:
1. Her DD Form 214, Certificate of Release or Discharge from Active Duty
be changed in block 11 to reflect AFSC 20CO, Logistics Commander, two years
five months; C21R3, Commander Logistics Readiness, one year; C21L3,
Logistician, two years eleven months; 21A3, Aircraft Maintenance and
Munitions, four years two months; 21T4, Transportation Staff, three years;
6054; Transportation Officer, five years five months and AFSC 21M3.
2. Her DD Form 214, block 17, reflect "Yes" indicating she had a dental
exam.
3. Her DD Form 214, block 13, reflect a Meritorious Service Medal (MSM),
forth oak leaf cluster.
4. Her DD Form 214, block 14, reflect her military formal training courses
and certifications.
5. Her DD Form 214, block 20, reflect copy 6 was sent to "AZ" Veterans
Affairs and the box be marked "Yes."

APPLICANT CONTENDS THAT:
Her DD Form 214 has incorrect and/or incomplete information.
In support of her request, the applicant submits a personal statement,
copies of her DD Form 214, retirement orders, copies of letters from HQ
AFPC/DFFCMP, HQ AFPC/DPSRP, HQ AFPC/DPSOMP, DD Form 214 worksheet, DD Form
214 inquiry, and other documentation extracted from her military personnel
records.
Her complete submission, with attachments, is at Exhibit A.

STATEMENT OF FACTS:
The applicant was commissioned in the Regular Air Force on 4 November 1986.
On 30 November 2006, she was retired in the grade of Lieutenant Colonel
with service characterized as honorable.
She served a total of 20 years and 27 days on active duty.
The applicant's requests regarding her MSM, formal training, and Veteran
Administration were administratively corrected.

AIR FORCE EVALUATION:
HQ AFPC/DPAMD approved her request to change her DD Form 214 to reflect she
had a dental exam. Her DD Form 214 was administratively corrected. DPAMD
states the applicant's DD Form 214, line 17 was marked “No” indicating she
was not provided a complete dental exam and all the appropriate dental
services and treatment within 90 days of separation. She provided a sign
memorandum from the MPF stating the dental exam was provided within 90 days
of separation/retirement and appropriate dental services/treatment was
provided by the Armed Forces, therefore line 17 should be changed to Yes.
The complete DPAMD evaluation is attached at Exhibit C.
AFPC/DPSIDC recommends denial of her request to include the requested AFSCs
on the DD Form 214. DPSIDC states neither the documentation the applicant
originally provided nor other documentation submitted as a result of their
requests on 18 October 2007 for additional information support the
corrective action requested by the applicant. AFI 36-3202, Separation
Documents, states item 11 of the DD Form 214 will reflect the primary AFSC
and all additional AFSCs in which the member served for one year or more,
during member's continuous active military service. Furthermore, it will
reflect, for each specialty, the highest skill level in which the member
performed duties. According to her correspondence, although AFSC 21M3 was
recorded as a tertiary AFSC, she never actually perform duties in the
specific AFSC. She did, in fact perform duties as a munitions officer;
however, these duties were accounted for when recorded as AFSCs 21AX and
21LX, as currently evidenced on her DD Form 214, item 11. Although she was
awarded AFSC 21M3 as a tertiary, it was only done to facilitate the
tracking of members with munitions and maintenance experience.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
Copies of the Air Force evaluations were forwarded to the applicant on 4
April 2008 for review and comment within 30 days. As of this date, this
office has received no response (Exhibit E).

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law or
regulations.
2. The application was timely filed.
3. Insufficient relevant evidence has been presented to demonstrate the
existence of error or injustice. The applicant is requesting her DD Form
214 be corrected to reflect the AFSCs she performed in while on active
duty. After a thorough review of the available evidence and the
applicant’s complete submission, we are not persuaded that her records
(other than the administrative corrections already made) should be
corrected. We agree with the opinion and recommendation of AFPC/DPSIDC and
adopt its rationale as the basis for our conclusion that the applicant has
not provided sufficient documentation to support her request. Therefore, in
the absence of evidence to the contrary, we find no compelling basis to
recommend granting the relief sought in this application.

THE BOARD RECOMMENDS:
The applicant be notified that the evidence presented did not demonstrate
the existence of material error or injustice; that the application was
denied without a personal appearance; and that the application will only be
reconsidered upon the submission of newly discovered relevant evidence not
considered with this application.
