APPLICANT REQUESTS THAT:

Block 11 on her DD Form  214,  Certificate  of  Release  or  Discharge  from
Active Duty, be changed to read:

1.  M42G3, Medical Svc Specl, Physician Assistant, 1 year and 6 months.

2.  M42G3F, Medical Service Specl, PA, Oncology/Bone Marrow  Transplant,  10
years 2 months.

APPLICANT CONTENDS THAT:

She was stationed  at  Lackland  in  1997  where  she  started  Bone  Marrow
Transplant/Oncology  and  never  left  the   Oncology   Field   except   for
deployments.  This can be verified  by  the  medical  facility  Bone  Marrow
Transplant Director.

She recently received her DD Form 214 via mail.  She was on  terminal  leave
prior to her discharge on June 1st and was not able to view her DD Form  214
online.  She made corrections earlier but did not save.   She  had  no  idea
the form was still incorrect.

In support of the application, the applicant submits her DD Form 214.

STATEMENT OF FACTS:

On 21 Sep 95, the applicant was honorably discharged from  the  Regular  Air
Force in the grade of Staff Sergeant and commissioned in the same branch  of
service.

The following is a resume of the applicant's duty titles extracted from  her
Officer  Effectiveness  Reports  (OERs)  during  the  period  in   question,
commencing with the report ending 19 Apr 97:

The applicant was honorably retired from active duty on 31  May  2007.   She
served 24 years, 3 months and 22 days on active duty.

The remaining relevant facts pertaining to this application, extracted  from
the applicant’s military records, are contained in the  letter  prepared  by
the appropriate office of the Air Force at Exhibit C.

AIR FORCE EVALUATION:

HQ AFPC/DPSIDC recommends denial.  DPSIDC states they were  able  to  verify
from the applicant's performance reports that she performed  duties  in  Air
Force Specialty Codes (AFSCs) 42G3 and M42G3F.  However,  only  an  AF  Form
2096, Classification or On-the-Job Training Action, a  P-series  order,  PC-
III generated AFSC action notice, or a report  on  individual  person  (RIP)
can serve as source documents to award an AFSC – not performance reports.

Although members can only perform  duty  in  awarded  AFSCs,  DPSIDC  cannot
conclude from the documentation provided when the AFSCs  were  awarded.   In
absence of any of the supporting documents mentioned  above,  DPSIDC  states
they cannot support corrective action.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

In her response dated 15 Oct 07,  the  applicant  states  she  is  enclosing
documents to prove she served in AFSC M42G3F  as  of  1999.   She  tried  to
register for care at  the  VA  for  benefits  from  her  Operation  Enduring
Freedom (OEF) tour.  She was told that  she  will  not  get  these  benefits
until her DD Form 214 reflects the dates she spent in Afghanistan  prior  to
her retirement.

The applicant's complete response, with attachments, is at Exhibit E.

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided  by  existing  law  or
regulations.

2.  The application was timely filed.

3.  Insufficient relevant evidence has been  presented  to  demonstrate  the
existence of error injustice in regards to the applicant's rebuttal  request
for the inclusive dates of her tour in Afghanistan to be  indicated  on  her
DD Form 214.  Credible documentation exists in the applicant's records  that
show her time  served  in  Afghanistan.   Therefore,  this  portion  of  the
applicant's request is denied.

4.  Sufficient relevant evidence  has  been  presented  to  demonstrate  the
existence  of  error  or  injustice.   After   reviewing   the   applicant’s
submission, we believe that an error occurred when her  separation  document
was prepared.  We note the certificates of  completion  and  AF  Form  2096s
approved by the applicant's commander and conclude  that  they  substantiate
her claim that she served in the oncology field since 1997.   Therefore,  we
recommend that the records be corrected as indicated below.

THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air  Force  relating
to APPLICANT  be  corrected  by  amending  the  Certificate  of  Release  or
Discharge from Active Duty, DD Form 214, block 11, to read:  M42G3,  MEDICAL
SVC SPECL, PHYSICIAN  ASSISTANT,  1  YEAR  AND  6  MONTHS;  M42G3F,  MEDICAL
SERVICE SPECLST,  PA,  ONCOLOGY/BONE  MARROW  TRANSPLANT,  10  YEARS  AND  2
MONTHS.

MEMORANDUM FOR THE CHIEF OF STAFF

      Having received and considered the recommendation of the Air Force
Board for Correction of Military Records and under the authority of
Section 1552, Title 10, United States Code (70A Stat 116), it is directed
that:

      The pertinent military records of the Department of the Air Force
relating APPLICANT, be corrected by amending the Certificate of Release
or Discharge from Active Duty, DD Form 214, block 11, to read:  M42G3,
MEDICAL SVC SPECL, PHYSICIAN ASSISTANT, 1 YEAR AND 6 MONTHS; M42G3F,
MEDICAL SERVICE SPECLST, PA, ONCOLOGY/BONE MARROW TRANSPLANT, 10 YEARS
AND 2 MONTHS.
