APPLICANT REQUESTS THAT:

Her records be corrected  to  reflect  the  award  of  the  Air  Force
Achievement Medal (AFAM).

APPLICANT CONTENDS THAT:

This medal was not awarded in time to be added to her DD Form  214  at
the time of discharge.

In support of the appeal, applicant submits a personal statement,  the
letter received by the Veterans Affairs office, a copy of her DD  Form
214, and a copy of the AFAM citation awarded for the period 31 Oct  86
to 24 Jan 88.

Applicant's complete submission, with attachments, is at Exhibit A.

STATEMENT OF FACTS:

Applicant is a former enlisted member of the Air Force who  served  on
active duty from 22 Apr 85 to 24 Jan 88.

Her DD Form 214 issued in conjunction with her  24  Jan  88  discharge
indicates she was awarded no decorations.

AIR FORCE EVALUATION:

AFPC/DPPPR recommends disapproval based on the fact  that  no  special
orders were found in applicant’s military record to  substantiate  the
claim.

Since the special order is the official document that  authorizes  the
decoration, they are unable to verify her entitlement to the AFAM.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

On 23 May 06, a copy of the Air Force evaluation was forwarded to  the
applicant for review and response.  As of this date, no  response  has
been received by this office.

THE BOARD CONCLUDES THAT:

1.    The applicant has exhausted all remedies  provided  by  existing
law or regulations.

2.    The application was not timely filed;  however,  it  is  in  the
interest of justice to excuse the failure to timely file.

3.    Sufficient relevant evidence has been presented  to  demonstrate
the existence of error or injustice.  Applicant contends that she  was
awarded the AFAM as evidence by the  signed  Certificate/Citation  she
has provided; however, her copy of the special order awarding her  the
decoration was  destroyed  by  water  while  her  belongings  were  in
storage.  Based on the documentation provided, and  in  an  effort  to
remove any possibility of an injustice to the applicant, we  recommend
that her record be corrected to the extent indicated below.

THE BOARD RECOMMENDS THAT:

The pertinent military records of the  Department  of  the  Air  Force
relating to APPLICANT, be corrected to show  that  the  DD  Form  214,
Certificate of Release  or  Discharge  from  Active  Duty,  issued  in
conjunction with her 24 January 1988 separation, be  amended  in  Item
13,  Decorations,  Medals,  Badges,  Citations  and  Campaign  Ribbons
Awarded or Authorized, to include the Air Force Achievement Medal.

MEMORANDUM FOR THE CHIEF OF STAFF

      Having received and considered the recommendation of the Air
Force Board for Correction of Military Records and under the authority
of Section 1552, Title 10, United States Code (70A Stat 116), it is
directed that:

      The pertinent military records of the Department of the Air
Force relating to APPLICANT, be corrected to show that the DD Form
214, Certificate of Release or Discharge from Active Duty, issued in
conjunction with her 24 January 1988 separation, be, and hereby is,
amended in Item 13, Decorations, Medals, Badges, Citations and
Campaign Ribbons Awarded or Authorized, to include the Air Force
Achievement Medal.
