APPLICANT REQUESTS THAT:
She be awarded the Air Force Achievement Medal (AFAM).

APPLICANT CONTENDS THAT:
The AFAM is missing from her DD Form 214, Certificate of Release or
Discharge from Active Duty, dated 26 Feb 90.
In support of her appeal, applicant submitted a copy of her AFAM
certificate, dated 31 Mar 90.
Applicant’s complete submission, with attachment, is at Exhibit A.

STATEMENT OF FACTS:
Applicant enlisted in the Regular Air Force on 13 Dec 84. She was
progressively promoted to the rank of senior airman with a date of
rank of 13 Mar 88.
On 26 Feb 90, applicant was honorably released from active duty and
placed on the Temporary Disability Retirement List (TDRL) under the
provisions of AFR 35-4.
DD Form 214 listed award of the Air Force Good Conduct Medal
(AFGCM), Air Force Training Ribbon (AFTR), Air Force Overseas
Service Long Tour Ribbon, Air Force Outstanding Unit Award (AFOUA),
and the Air Force Longevity Service Award (AFLSA).
Applicant was permanently retired for disability effective
15 Sep 91, with a disability rating of 40%. She completed 5 years,
2 months, and 14 days of active service for retirement.

AIR FORCE EVALUATION:
HQ AFPC/DPPPR reviewed this application and recommended denial.
Applicant submitted a copy of what appears to be an official AFAM
certificate; however, it was not submitted with a special order.
Without the special order to validate the AFAM, DPPPR could not
confirm entitlement to the award.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the applicant
on 13 Jul 07 for review and comment within 30 days. As of this
date, no response has been received by this office (Exhibit D).

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing
law or regulations.
2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Sufficient relevant evidence has been presented to demonstrate
the existence of error or injustice. After a careful review of the
evidence of record and that provided by the applicant, the Board
noted the applicant provided a sealed copy of the AFAM certificate
to substantiate her claim. The Board considered the recommendation
of the Recognition Programs Team Lead; nonetheless, we believe to
remove the possibility of an injustice to the applicant that any
doubt should be resolved in her favor. Therefore, we recommend the
applicant’s record be corrected to the extent indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air Force
relating to APPLICANT, be corrected to show that her DD Form 214,
Certificate of Release or Discharge from Active Duty, issued on
26 February 1990, be amended in Section 26, Decorations, Medals,
Badges, Commendations, Citations, and Campaign Ribbons Awarded or
Authorized to reflect award of the Air Force Achievement Medal.

MEMORANDUM FOR THE CHIEF OF STAFF
Having received and considered the recommendation of the Air
Force Board for Correction of Military Records and under the
authority of Section 1552, Title 10, United States Code (70A Stat
116), it is directed that:
The pertinent military records of the Department of the Air
Force relating to XXXXXXXX, XXXXXXX, be corrected to show that her
DD Form 214, Certificate of Release or Discharge from Active Duty,
issued on 26 February 1990, be, and hereby is, amended in Section
26, Decorations, Medals, Badges, Commendations, Citations, and
Campaign Ribbons Awarded or Authorized to reflect award of the Air
Force Achievement Medal.
