APPLICANT REQUESTS THAT:
His DD Form 214 be corrected to reflect award of the Southwest Asia
Service Medal (SWASM).

APPLICANT CONTENDS THAT:
The SWASM he was awarded is not reflected on his DD Form 214. He
requests the SWASM be added to his DD Form 214.

STATEMENT OF FACTS:
The applicant enlisted in the Regular Air Force (RegAF) on 11 January
1978, as an airman basic (AB) for a period of four years.
His DD Form 214 reflects he was awarded the Air Force Outstanding Unit
Award (AFOUA), Air Good Conduct Medal (AFGCM) with three Oak Leaf
Clusters (OLC), National Defense Service Medal (NDSM), Air Force
Overseas Long Tour Ribbon; Air Force Longevity Service Award with 2
OLCs and the Air Force Training Ribbon.
The applicant’s DD Form 214 reflects he had 4 years, 2 months and 22
days of foreign service. His DD Form 214 also reflects he served from
2 August 1990 to 15 June 1992 in support of Operation DESERT
SHIELD/STORM.
The applicant was honorably released from active duty on 15 June 1992
and transferred to the Air Force Reserve. He served 14 years, 5
months and 5 days of active service.
On 26 May 2004,AFPC/DPPPR requested the applicant provide a copy of
his TDY order and travel voucher to verify his eligibility for the
award of the SWASM. The applicant did not respond.

AIR FORCE EVALUATION:
HQ AFPC/DPPPR states servicemembers are awarded the SWASM for service
in support of Operation DESERT SHIELD/DESERT STORM from 2 August 1990
through 30 November 1995. The TDY order and Travel voucher are used
to determine a servicemember’s eligibility for the award. The TDY
order shows which operation the servicemember was deployed and the
voucher verifies the days the servicemember was in the Area of
Responsibility. HQ AFPC/DPPPR further states since the applicant did
not respond to the request to provide documentation to support his
request, they are unable to verify his eligibility for the SWASM.
DPPPR recommends denying the requested relief.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the applicant on 4
June 2004, for review and response. As of this date, no response has
been received by this office.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing
law or regulations.
2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Insufficient relevant evidence has been presented to demonstrate
the existence of error or injustice. We took notice of the
applicant's complete submission in judging the merits of the case;
however, we agree with the opinion and the recommendation of the Air
Force and adopt their rationale as the basis for our conclusion that
the applicant has not been the victim of an error or an injustice.
The applicant did not respond to a request to provide a copy of his
travel voucher or TDY orders showing he was TDY or deployed in support
of Desert Shield/Desert Storm. Therefore, in the absence of evidence
to
the contrary, we find no compelling basis to recommend granting the
relief sought in this application.

THE BOARD DETERMINES THAT:
The applicant be notified that the evidence presented did not
demonstrate the existence of material error or injustice; that the
application was denied without a personal appearance; and that the
application will only be reconsidered upon the submission of newly
discovered relevant evidence not considered with this application.
