APPLICANT REQUESTS THAT:

The closeout date of the Officer Performance Report (OPR) rendered  for  the
period 19 October 1999 through 1 September 2000, be  changed  to  1  October
2000 and that it be filed in his records.

APPLICANT CONTENDS THAT:

The contested report should be filed in his records since it  documents  his
last year of honorable service.

The applicant states that the report  was  due  to  his  Military  Personnel
Flight (MPF) by 1 October 2000 to ensure it was included  in  his  permanent
record; however, despite his numerous phone calls to the rater,  the  report
was not signed until 11 April 2001, after his separation.  As a  result,  it
was never filed in his  records  and  his  last  year  of  honorable  active
service is not documented.  The applicant notes that accurate  documentation
of his active duty military service is vital to future  Guard,  Reserve,  or
civil job opportunities.

The applicant also states that the report should have a closeout date  of  1
October 2000, the actual date of his departure.

In support of the appeal, applicant submits a copy of the  contested  report
and an OPR Notice indicating projected departure and separation dates  of  1
October 2000.

STATEMENT OF FACTS:

The applicant was commissioned a second lieutenant in the Regular Air  Force
on 29 May 1991 and entered  extended  active  duty.   He  was  progressively
promoted to the grade of captain, effective 29 May 1995.

The applicant was honorably discharged on 1 October 2000.   He  completed  a
total of 9 years, 4 months, and 3 days of active service.

AIR FORCE EVALUATION:

AFPC/DPPPE recommends partial relief.  AFPC/DPPPE states, in part, that  the
OPR covers a significant  portion  of  time  and  should  be  filed  in  the
applicant’s permanent record; however, they recommend denial of his  request
to change the  closing  date  of  the  report  to  1  October  2000.   While
commanders have the authority to extend the date  of  a  report  to  include
significant events, the applicant has not shown any significant events  took
place during his last month of Air  Force  service  or  that  any  important
information would be documented by extending the closeout date.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

A complete copy of the Air Force evaluation was forwarded to  the  applicant
on 9 November 2001, for review and response.  As of this date,  no  response
has been received by this office.

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided  by  existing  law  or
regulations.

2.  The application was timely filed.

3.  Sufficient relevant evidence  has  been  presented  to  demonstrate  the
existence of probable error or injustice warranting partial  relief  of  the
applicant’s requests.  We  noted  the  applicant’s  complete  submission  in
judging the merits  of  the  case  and  we  agree  that  the  OPR  covers  a
significant portion of time and should be included in his permanent  record.
 However, the request to change the period of the  report  from  19  October
1999 through 1 September 2000 to 19 October  1999  through  1 October  2000,
which is the date of his separation from active duty, is  denied.   In  this
respect, we note that through dates are set 30 days prior  to  the  date  of
separation.  Therefore, the applicant’s record should be  corrected  to  the
extent indicated below.

THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air  Force  relating
to  APPLICANT,  be  corrected  to  show  that  the  Company  Grade   Officer
Performance Report, AF Form 707B, rendered for the  period  19 October  1999
through 1 September 2000, was accepted for file on or  before  30  September
2000.

MEMORANDUM FOR THE CHIEF OF STAFF

      Having received and considered the recommendation of the Air Force
Board for Correction of Military Records and under the authority of Section
1552, Title 10, United States Code (70A Stat 116), it is directed that:

      The pertinent military records of the Department of the Air Force
relating to APPLICANT be corrected to show that the Company Grade Officer
Performance Report, AF Form 707B, rendered for the period 19 October 1999
through 1 September 2000, was accepted for file on or before 30 September
2000.
