THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, reconsideration of his previous request for correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he was awarded a second Army Achievement Medal.

2.  The applicant states at the time of his discharge, he did not have the Army Achievement Medal.

3.  The applicant provides:

* DD Form 214, dated 18 November 2011
* DA Form 638 (Recommendation for Award), dated 26 August 2009
* DA Form 638, dated 3 November 2009

CONSIDERATION OF EVIDENCE:

1.  Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20130015621 on 14 May 2014.

2.  As new evidence, the applicant provides a copy of a DA Form 638 dated 3 November 2009, which shows he was awarded the Army Achievement Medal with one Oak Leaf Cluster for meritorious service during the period
17 December 2008 through 15 December 2009 in Permanent Order Number 313-4.
DISCUSSION AND CONCLUSIONS:

1.  The applicant's contentions have been noted.  His supporting evidence has been considered.

2.  The available evidence substantiates he was awarded the Army Achievement Medal with one Oak Leaf Cluster for meritorious service during the period 
17 December 2008 through 15 December 2009 in Permanent Order Number 313-4.  His DD Form 214 should be amended to show his second award of the Army Achievement Medal.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant amendment of the ABCMR’s decision in Docket Number AR20130015621, dated 14 May 2014.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 to add one Oak Leaf Cluster to be affixed to his already-awarded Army Achievement Medal.
