THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show award of the Army Achievement Medal with 2nd Oak Leaf Cluster (3rd Award).

2.  The applicant states he received three Army Achievement Medals.

3.  The applicant provides copies of three Army Achievement Medal Certificates with corresponding DA Forms 638 (Recommendation of Award). 

CONSIDERATION OF EVIDENCE:

1.  The applicant's records show he enlisted in the Regular Army on 24 April 2007 and he was honorably released from active duty on 23 April 2011.

2.  His DD Form 214 lists his authorized awards as the:

* Army Commendation Medal
* Army Achievement Medal (2nd Award)
* Army Good Conduct Medal
* National Defense Service Medal
* Global War on Terrorism Service Medal 
* Iraq Campaign Medal with campaign star
* Army Service Ribbon
* Overseas Service Ribbon 

3.  He provides:

   a.  A DA Form 638, which shows Permanent Orders Number 117-07, issued by the 554th Engineer Battalion, Fort Leonard Wood, MO, dated 27 April 2009, awarded him the Army Achievement Medal for outstanding achievement for the period 20 April 2009 to 22 April 2009.

   b.  A DA Form 638, which shows Permanent Orders Number 260-01, issued by Headquarters, 94th Engineer Battalion, Fort Leonard Wood, MO, dated 
17 September 2010, awarded him the Army Achievement Medal with 1st Oak Leaf Cluster for meritorious service for the period 11 September 2010 to 
17 September 2010.

   c.  A DA Form 638, which shows Permanent Orders Number 083-01, issued by Headquarters, 94th Engineer Battalion, Fort Leonard Wood, MO, dated        24 March 2011, awarded him the Army Achievement Medal with 2nd Oak Leaf Cluster for meritorious service for the period 1 April 2009 to 23 April 2011.

DISCUSSION AND CONCLUSIONS:

1.  The applicant requests his DD Form 214 be corrected to show the Army Achievement Medal with 2nd Oak Leaf Cluster (3rd Award).

2.  Permanent orders awarded him the Army Achievement Medal with 2nd Oak Leaf Cluster which is not shown on his DD Form 214.  Therefore, he is entitled to correction of his DD Form 214 to show this award.

3.  In view of the above, his request should be granted.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the Army Achievement Medal (2nd Award) from his DD Form 214 and adding the Army Achievement Medal (3rd Award).
