THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests inclusion of his period of service in Afghanistan from 23 June 2007 through 5 August 2007 on his DD Form 214 (Certificate of Release or Discharge from Active Duty).

2.  The applicant states his service time in Afghanistan is not shown on his DD Form 214.  His period of service in Afghanistan is not shown since his orders were open for the entire area under Operation Iraqi Freedom/Enduring Freedom. He has been told that the lack of specific location information could affect current or future Department of Veterans Affairs benefits.

3.  The applicant provides:

* DD Forms 214 for the periods ending in October 2007, December 2010, and January 2015
* Afghanistan Campaign Medal award memorandum
* DA Form 2-1 (Personnel Qualification Record)
* North Atlantic Treaty Organization (NATO) Medal award certificate
* Afghanistan Campaign Medal award certificate

CONSIDERATION OF EVIDENCE:

1.  The applicant is an active drilling Reserve staff sergeant.

2.  He is shown to have been called to active duty in support of Operation Iraqi Freedom/Enduring Freedom from 24 June 2006 through 2 October 2007, 23 September 2009 through 8 December 2010, and 30 May 2013 through 1 January 2015.

3.  The applicant's 2 October 2007 DD Form 214 shows "SERVICE IN IRAQ/KUWAIT 20060906-20070908" and award of the Army Commendation Medal, Army Good Conduct Medal, Army Reserve Components Achievement Medal, National Defense Service Medal, Afghanistan Campaign Medal, Global War Terrorism Expeditionary Medal, Global War On Terrorism Service Medal, Army Service Ribbon, Overseas Service Ribbon, and the Armed Forces Reserve Medal with "M" Device.

4.  On 6 August 2007, the 335th Theater Signal Command, Camp Arifjan, Kuwait awarded the applicant the Afghanistan Campaign Medal for his service from 27 June 2007-5 August 2007.

5.  The applicant was also awarded the Non Article 5 NATO Medal for service with NATO in support of the International Security Assistance Force Operation in Afghanistan during the period 27 June 2007-5 August 2007.

6.  The applicant's DA Form 2-1 shows foreign service in —

* Kuwait from 6 September 2006 through 6 September 2007
* Afghanistan from 23 June 2007 through 5 August 2007
* Iraq from 17 December 2009 through 12 September 2010
* Korea from 17 August 2012 through 2 September 2012

7.  Army Regulation 635-5 (Separation Documents), then in effect, prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army.  It establishes standardized policy for preparing and distributing the DD Form 214 and states block 18 (REMARKS) is to be used for Headquarters, Department of the Army mandatory requirements when a separate block is not available and as a continuation for entries in blocks 11, 13, and 14.  For a Reserve Soldier ordered to active duty and deployed to a foreign country, enter the following three statements in succession. 

     a.  "ORDERED TO ACTIVE DUTY IN SUPPORT OF (OPERATION NAME) PER 10 USC (applicable section)." 
     
     b.  "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)." 
     
     c.  "SOLDIER COMPLETED PERIOD FOR WHICH ORDERED TO ACTIVE DUTY FOR PURPOSE OF POST-SERVICE BENEFITS AND ENTITLEMENTS." 

DISCUSSION AND CONCLUSIONS:

1.  The applicant's records show he was ordered to active duty for the period 24 June 2006 through 2 October 2007 and that during this period he served in Iraq, Kuwait, and Afghanistan.  

2.  His 2007 DD Form 214 at block 18 shows the entry "SERVICE IN IRAQ/KUWAIT 20060906-20070908."

3.  Since his deployment is shown to have been in three countries not just the two noted on the DD Form 214 and it was a continuous deployment, it would be appropriate to correct the 2 October 2007 DD Form 214 to show at block 18 "SERVICE IN IRAQ/KUWAIT/AFGHANISTAN 20060906-20070908."

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending the 2 October 2007 DD Form 214 to show at block 18 "SERVICE IN IRAQ/KUWAIT/AFGHANISTAN 20060906-20070908."
