THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests her award of the Army Commendation Medal be added to her DD Form 214 (Certificate of Release or Discharge from Active Duty) with a separation date of 20 July 2014.

2.  The applicant states the award did not receive final approval until after she had been released from active duty.

3.  The applicant provides:

* her DD Form 214
* DA Form 638 (Recommendation for Award), dated 21 May 2014
* Army Commendation Medal Certificate

CONSIDERATION OF EVIDENCE:

1.  On 21 July 2010, she enlisted in the Regular Army.  She was assigned to the Headquarters and Headquarters Battalion, 1st Cavalry Division at Fort Hood, TX.

2.  On 20 July 2014, she was released from active duty.  Her DD Form 214 does not show the award of the Army Commendation Medal.

3.  A DA Form 638, dated 21 May 2014, shows her platoon sergeant recommended her for the award of the Army Commendation Medal for exceptionally meritorious service while serving in multiple positions and volunteer efforts from 20 April 2012 to 20 July 2014.  Her chain of command recommended approval and on 9 September 2014, the Commander, 1st Brigade Combat Team, 
1st Calvary Division approved the award of the Army Commendation Medal.

4.  An Army Commendation Medal Certificate shows the award was announced in Headquarters, 1st Brigade Combat Team, 1st Calvary Division Permanent Order Number 239-003, dated 27 August 2014.

DISCUSSION AND CONCLUSIONS:

The DA Form 638 and her Army Commendation Medal Certificate are sufficient evidence to show she was awarded the Army Commendation Medal.  Therefore, the award should be added to her DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

amending her DD Form 214 with a separation date of 20 July 2014 to add the Army Commendation Medal.
