THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Meritorious Service Medal (MSM) and the Army Good Conduct Medal (AGCM) (5th Award).  

2.  The applicant states he was awarded the MSM and the 5th award of the AGCM but the certificates and the orders were completed after his retirement date.

3.  The applicant provides a DD Form 214 for the period ending 26 April 2013, orders for the AGCM (5th Award), DA Form 638 (Recommendation for Award), and an MSM Certificate.  

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 17 March 1998.  On 16 March 2001, he was honorably released from active duty and transferred to the U.S. Army Reserve.  On 8 March 2002, he reenlisted in the Regular Army for a period of 3 years.  

2.  A DA Form 638 shows he was awarded the MSM for service during the period 27 April 2003 to 26 April 2013 by Permanent Orders Number 072-015, dated 
1 April 2013, issued by Headquarters, 1st Cavalry Division, Fort Hood, TX.

3.  He was transferred to the Temporary Disability Retired List on 26 April 2013.  His DD Form 214 for this period of service shows in:
	a.  block 13 (Decorations, Medal, Badges, Citations and Campaign Ribbons Awarded or Authorized) the:

* Iraq Campaign Medal with four campaign stars (correctly known as the Iraq Campaign Medal with four bronze service stars)
* Bronze Star Medal (2nd Award)
* Army Commendation Medal (3rd Award)
* Army Achievement Medal (2nd Award)
* Meritorious Unit Commendation (2nd Award)
* Valorous Unit Award
* Army Good Conduct Medal (4th Award)
* National Defense Service Medal
* Global War on Terrorism Expeditionary Medal
* Global War on Terrorism Service Medal
* Iraq Campaign Medal with campaign star (5th Award) (interpreted to mean the Iraq Campaign Medal with one silver service star)
* Noncommissioned Officer Development Ribbon (3rd Award)
* Army Service Ribbon
* Overseas Service Ribbon (3rd Award)
* Combat Action Badge
* Basic Marksmanship Qualification Badge
* Driver and Mechanic Badge with T, W, and Mechanic component bars 

	b.  block 18 (Remarks) he served in Iraq during the following periods:

* 8 March 2004 - 6 March 2005
* 20 October 2006 - 6 January 2008
* 8 January - 12 December 2009
* 26 May - 16 November 2011

4.  Permanent Orders Number 122-051, dated 1 May 2013, issued by Headquarters, 2nd Brigade Combat Team, 1st Calvary Division, Fort Hood, TX awarded him the AGCM (5th Award).

5.  Army Regulation 600-8-22 (Military Awards) states:

	a.  Only one award of the Iraq Campaign Medal may be authorized for any individual.  

	b.  A bronze service star is authorized for wear on the Iraq Campaign Medal for participation in each credited campaign.  A silver service star is authorized in lieu of five bronze service stars.  Approved campaigns are:
* Liberation of Iraq (19 March 2003-1 May 2003)
* Transition of Iraq (2 May 2003-28 June 2004)
* Iraqi Governance (29 June 2004-15 December 2005)
* National Resolution (16 December 2005-9 January 2007)
* Iraqi Surge (10 January 2007-31 December 2008)
* Iraqi Sovereignty (1 January 2009-31 August 2010)
* New Dawn (1 September 2010-31 December 2011)

DISCUSSION AND CONCLUSIONS:

1.  The applicant's contention that his DD Form 214 should be corrected to show the MSM and the 5th award of the AGCM has been carefully considered.  

2.  Orders show he was awarded the MSM and AGCM (5th Award).  Therefore, these awards should be added to his DD Form 214.

3.  He was awarded the Iraq Campaign Medal with four bronze service stars.  However, he participated in five campaigns during his service in Iraq.  Therefore, he is authorized one silver service star for wear on the Iraq Campaign Medal and correction of is DD Form 214 to reflect this silver service star.  

4.  Block 13 of his DD Form 214 includes the Iraq Campaign Medal with four campaign stars and the Iraq Campaign Medal with campaign star (5th award).  Because only one award of the Iraq Campaign Medal is authorized to any individual, the entry that reads "Iraq Campaign Medal with campaign star (5th Award)" is a duplicate entry that should be removed from his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting from block 13 of his DD Form 214 the Iraq Campaign Medal with four campaign stars and the Iraq Campaign Medal with campaign star (5th Award); and

	b.  adding to block 13 of his DD Form 214 the following:

* Meritorious Service Medal
* Army Good Conduct Medal (5th Award)
* Iraq Campaign Medal with one silver service star
