THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show his two awards of the Meritorious Service Medal (MSM).

2.  The applicant states he was awarded a second MSM at the time of his retirement that is not reflected on his DD Form 214.

3.  The applicant provides a copy of his MSM (2nd Award).

CONSIDERATION OF EVIDENCE:

1.  The applicant served in the Regular Army as a military police in the pay grade of E-6.  He served two tours in Korea, one tour in Germany, and three tours in Iraq.  He was awarded his first MSM in Permanent Orders Number 024-011 issued by Headquarters, U.S. Army Training Center and Fort Jackson on 1 February 2002.

2.  Permanent Orders Number 204-01, issued by Headquarters, III Corps and Fort Hood, Texas on 15 August 2014 awarded him the MSM (2nd Award).

3.  On 31 August 2014, the applicant was honorably retired by reason of length of service.  His DD Form 214 issued at the time of his retirement shows he was awarded the "Meritorious Service Medal."


DISCUSSION AND CONCLUSIONS:

1.  The applicant was awarded the MSM (2nd Award); however, at the time of his retirement his DD Form 214 was incorrectly prepared to reflect the "Meritorious Service Medal."

2.  Accordingly, his DD Form 214 should be corrected to show his award of the MSM (2nd Award).

BOARD DETERMINATION/RECOMMENDATION:

1. The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting the award of the MSM from his DD Form 214 and adding the MSM (2nd Award).

2.  The Board wants the applicant and all others to know that the sacrifices he made in service to the United States during the Global War on Terrorism are deeply appreciated.  The applicant and all Americans should be justifiably proud of his service in arms.
