THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Meritorious Service Medal with First Oak Leaf Cluster (MSM w/1st OLC).

2.  The applicant states his second MSM was not reflected in the Interactive Personnel Electronic Records Management System (iPERMS).
   
3.  The applicant provides

* DD Form 214
* MSM Certificate and Order

CONSIDERATION OF EVIDENCE:

1.  The applicant's military record shows he last entered active duty from the Army National Guard on 20 July 2011 and served until he was retired from active duty in the rank of sergeant first class (SFC/E-7) on 31 July 2013.  He completed a total of 24 years, 11 months, and 21 days of creditable active duty service.

2.  His retirement DD Form 214 includes an entry which shows he earned the MSM.

3.  The applicant's iPERMS record contains and he provides Headquarters, Iowa National Guard, Office of the Adjutant General, Permanent Order Number 067-001, dated on 8 March 2013.  It announced his award of the MSM w/OLC, for exceptional meritorious service during the period 23 April 1982 to 31 July 2013.

DISCUSSION AND CONCLUSIONS:

The evidence of record and the applicant's evidence clearly and convincingly show he was awarded a second MSM for over 24 years of exceptionally meritorious service culminating in his retirement from active duty on 31 July 2013.  Accordingly, his records should be corrected to show the MSM with 1st OLC.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

   a.  deleting from his DD Form 214 for the period ending 31 July 2013 the Meritorious Service Medal and replacing it with the Meritorious Service Medal (2nd Award); and
   
   b.  issuing a correction to his retirement DD Form 214 to reflect this award.
