THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show his fourth award of the Army Achievement Medal (AAM 4th Award).
2. The applicant states, in effect, he was awarded four awards of the AAM; however, his DD Form 214 reflects only three awards.
3. The applicant provides copies of his DD Form 214 and award of the AAM with three oak leaf clusters.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 10 April 2008 for a period of 5 years and training as a military police. He completed his one-station unit training at Fort Leonard Wood, Missouri and remained assigned to Fort Leonard Wood. He was promoted to the pay grade of E-5 on 1 May 2011 and he deployed to Iraq during the period 20100228 20110215.
2. On 9 April 2013, he was honorably released from active duty (REFRAD) due to the expiration of his term of service. His DD Form 214 shows he was awarded the Army Commendation Medal, AAM (3rd Award), Army Good Conduct Medal, National Defense Service Medal, Global War on Terrorism Service Medal, Iraq Campaign Medal with campaign star, Army Service Ribbon, Overseas Service Ribbon, and Driver and Mechanic Badge with special operator clasp.
3. The AAM award provided by the applicant shows that he was awarded the AAM with three oak leaf clusters in Permanent Orders Number 16-002 issued by Headquarters, 92nd Military Police Battalion on 17 January 2013.
DISCUSSION AND CONCLUSIONS:
1. The applicants contention that his DD Form 214 should reflect four awards of the AAM has been noted and found to have merit.
2. The applicant was awarded the fourth award of the AAM on 17 January 2013 and it is not reflected on his DD Form 214.
3. Accordingly, his DD Form 214 should be corrected to reflect that award.
