THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) by adding another Army Commendation Medal (ARCOM) and a 4th Army Achievement Medal (AAM).

2.  The applicant states the omission of these awards was an error.

3.  The applicant provides copies of approved DA Forms 638 (Recommendation for Award) with order numbers for the ARCOM, ARCOM with "V" Device, and four different AAMs.

CONSIDERATION OF EVIDENCE:

1.  The applicant served on active duty as a health care specialist from 19 October 2010 through 16 April 2014, a period 3 years, 5 months, and 28 days.  He was honorably released from active duty and transferred to the U.S. Army Reserve as a specialist (pay grade E-4).

2.  His DD Form 214 shows that he is authorized the Afghanistan Campaign Medal with campaign star, ARCOM with "V" Device, AAM (3rd Award), Army Good Conduct Medal, National Defense Service Medal, Global War on Terrorism Service Medal, Iraq Campaign Medal with campaign star, Army Service Ribbon, Overseas Service Ribbon (2nd Award), North Atlantic Treaty Organization (NATO) Medal, and Certificate of Achievement (2nd Award).


3.  His Interactive Personnel Electronic Records Management System (iPERMS)
record contains only the orders awarding him the ARCOM, one AAM, and the Army Good Conduct Medal.

4.  In support of his request the applicant submitted copies of: 

	a.  approved DA Form 638 showing he was awarded the ARCOM for meritorious service during the period 11 February 2013 to 9 November 2013;

	b.  approved DA Form 638 showing he was awarded the ARCOM with "V" Device for heroic action on 19 September 2013; and 

	c.  approved DA Forms 638 showing he was awarded the AAM for the period dated – 

* from 31 May 2011 to 12 June 2011
* from 18 August  2011 to 31 December 2011
* from 31 May 2013 to 1 July 2013
* from 6 May 2011 to 16 May 2014

5.  Army Regulation 600-8-22 (Military Awards) states an oak leaf cluster is awarded to denote the second and succeeding awards of certain decorations, among which is the ARCOM.

DISCUSSION AND CONCLUSIONS:

1.  The applicant requests that his second ARCOM and a 4th AAM be added to his DD Form 214.

2.  He was awarded an ARCOM with "V" Device and a second ARCOM.

3.  The applicant was awarded four AAMs.

4.  His records should be corrected as recommended below.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by changing his DD Form 214 by: 

	a.  deleting the Army Commendation Medal with "V" Device and the Army Achievement Medal (3rd Award); and 

	b.  entering the Army Commendation Medal with "V" Device and one Oak Leaf Cluster and the Army Achievement Medal (4th Award).
