THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty), with a separation date of 18 August 1995, to show her last name as "Gxx Arrxxxgui."
2. The applicant states the correct spelling of her last name is "Gxx Arrxxxgui." She also wants her full middle name to be shown on her DD Form 214. She states this correction is for personal use and for identification/usage with other agencies.
3. The applicant provides her State of California Identification Card, her Department of Veterans Affairs (VA) identification card, and her social security card.
CONSIDERATION OF EVIDENCE:
1. On 18 May 1993, she enlisted in the U.S. Army Reserve. Her DD Form 4 (Enlistment/Reenlistment Document Armed Forces of the United States) shows her last name as "Gxxarrxxxgui." Her DD Form 4 does not show her full middle.
2. Her DA Form 2-1 (Personnel Qualification Record) shows her last name as "Gxx-Arrxxxgui" and only her middle initial is entered.
3. Documents throughout her Military Personnel Records Jacket (MPRJ) do not show her full middle name as she requested. Her last name is shown throughout her MPRJ as:

* Gxx-Arrxxxgui
* Gxx Arrxxxgui
4. On 18 August 1995, she was discharged. Her DD Form 214 shows her last name as "Gxxarrxxxgtui." Her full middle name is not shown.
5. The documents she submitted show her last name as "Gxx." They also show her full middle name.
6. Army Regulation 635-5 (Separation Documents) at the time established the standardized policy for preparing and distributing the DD Form 214. The regulation directs that the purpose of the separation document is to provide the individual with documentary evidence of his or her military service at the time of release from active duty, retirement, or discharge. It is important that information entered on the form be complete and accurate and reflects the conditions as they existed at the time of separation.
DISCUSSION AND CONCLUSIONS:
1. Her last name on her DD Form 214 is misspelled and must be corrected. Because her last name appears in her MPRJ at times with a hyphen, or a space, or neither, there is no harm in correcting her last name to read "Gxx Arrxxxgui."
2. She did not enlist using her full middle name and her full middle name is not shown anywhere in her MPRJ; however, she now requests to change the middle name shown on her DD Form 214 to show her full middle name.
3. For historical purposes, the Army has an interest in maintaining the integrity of its records. The data and information contained in those records should reflect the conditions and circumstances that existed at the time the records were created. In the absence of a showing of material error or injustice, there is a reluctance to recommend that those records be changed. While it is understandable that she now desires to record her full middle name in her military records, there is not a sufficiently compelling reason for compromising the integrity of the Armys records at this late date.
4. The applicant is advised that a copy of this decisional document, which confirms her full middle name, will be filed in her MPRJ. This should serve to clarify any questions or confusion in regard to the difference in her middle name recorded in her military record and to satisfy her desire to have her full middle name documented in her MPRJ.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending her DD Form 214, with a separation date of 18 August 1995, to show her last name as requested on her application.
2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to her middle name.
