THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests preparation of a DA Form 1059 (Service School Academic Evaluation Report) for the Warrior Leader Course (WLC) and placement in her Army Military Human Resource Record (AMHRR).
2. The applicant states:
a. Her DA Form 1059 from the WLC is not filed in her AMHRR, but she does have the course certificate. She attended the WLC in Kosovo in 2010.
b. She was told by the school house they did not have a copy of her DA Form 1059; they are only required to keep them for 2 years and there was no way for them to reconstruct it. She was also told the "G" notation in the Army Training Requirements Reporting System and the copy of her certificate should suffice as documentation for graduation.
3. The applicant provides:

CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Army National Guard on 22 January 2001.
2. She provided a diploma, dated 4 March 2010, which shows she completed the WLC in Kosovo.
3. Item 17 (Civilian Education and Military Schools) of her DA Form 2-1 (Personnel Qualification Record Part II) also shows she completed the 2-week WLC in Kosovo in 2010.
4. She was promoted to staff sergeant on 5 October 2012.
5. A review of the applicant's AMHRR in the interactive Personnel Electronic Records Management System did not reveal a copy of a DA Form 1059 for the WLC. Her diploma for the WLC is filed in her AMHRR.
DISCUSSION AND CONCLUSIONS:
1. The evidence shows the applicant completed the WLC in Kosovo on 4 March 2010; however, her AMHRR does not contain the DA Form 1059 for this course.
2. Her request to recreate a DA Form 1059 was carefully considered. However, this Board corrects military records but normally cannot recreate a document that is not available in any form. Re-creating a DA Form 1059 would require knowing, in part, if she exceeded, achieved, or marginally achieved course standards; whether each of five demonstrated abilities (e.g., written communication) were not evaluated or evaluated as unsatisfactory, satisfactory, or superior.
