THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests the DA Form 1059 (Service School Academic Evaluation Report), dated 14 March 2006, which shows she attended the Warrior Leader Course (WLC) during the period 13 February 2006 to 14 February 2006 be removed from her official military personnel file (OMPF). 2. The applicant states: * the correct DA Form 1059 for this course shows she attended the course during the period 13 February 2006 through 14 March 2006 * the error was caused due to a lack of attention to detail by her small group leader * she wants to correct her records because she is coming up for her first consideration by the sergeant first class promotion board 3. The applicant provides: * DA Form 1059, dated 14 March 2006, which shows she attended WLC during the period 13 February 2006 through 14 February 2006 * DA Form 1059, dated 14 March 2006, which shows she attended WLC during the period 13 February 2006 through 14 March 2006 CONSIDERATION OF EVIDENCE: 1. She enlisted in the Regular Army on 20 November 2002 and has remained on active duty through continuous reenlistments. She was promoted to staff sergeant on 1 January 2009. 2. She provided a DA Form 1059, dated 14 March 2006, which shows she attended WLC during the period 13 February 2006 through 14 February 2006 and she achieved course standards. 3. She also provided a DA Form 1059, dated 14 March 2006, which shows she attended WLC during the period 13 February 2006 through 14 March 2006 and she achieved course standards. 4. A review of the performance section of her OMPF in the interactive Personnel Electronic Records Management System revealed a copy of both DA Forms 1059, dated 14 March 2006. 5. Information obtained from the internet states that WLC is taught at a Noncommissioned Officer (NCO) Academy in a live-in environment. You must remain at the NCO Academy for the duration of the course (Active Component – 4 weeks, Reserve Component – 2 weeks). DISCUSSION AND CONCLUSIONS: 1. She contends the correct DA Form 1059 shows she attended WLC during the period 13 February 2006 through 14 March 2006 and she provided a DA Form 1059 to support this contention. 2. Since the duration of WLC is 4 weeks for Regular Army Soldiers and evidence shows she attended WLC during the period 13 February 2006 to 14 March 2006 and achieved course standards, the DA Form 1059 which shows she attended WLC for 2 days should be removed from her OMPF.
