THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Joint Service Commendation Medal (JSCM).

2.  The applicant states the above award was finalized after he was separated and/or his DD Form 214 was processed.

3.  The applicant provides a copy of his JSCM Certificate.

CONSIDERATION OF EVIDENCE:

1.  The applicant executed an oath of office on 28 December 2005 as a second lieutenant.  On 6 January 2006, he was ordered to active duty.  His area of concentration was 12A (Engineer, General).

2.  On 10 December 2011, the applicant tendered his unqualified resignation due to completion of required active service.

3.  On 22 February 2012, the applicant's separation was approved with an effective date of 15 May 2012.

4.  On 15 May 2012, the applicant was discharged accordingly.  His DD Form 214 shows he completed 6 years, 4 months, and 10 days of creditable active service and does not show award of the JSCM.
5.  The applicant provides a copy of his JSCM Certificate which shows the medal was awarded by General Order Number D7-1288-12.

6.  Army Regulation 635-5 (Separation Documents) establishes standardized policy for preparing and distributing the DD Form 214.  The DD Form 214 provides a clear-cut record of active service at the time of release from active duty, retirement or discharge.  It further states to list awards and decorations for all periods of service in priority sequence as specified in Army Regulation 
600-8-22 and that each entry will be verified by the Soldier's record.

DISCUSSION AND CONCLUSIONS:

General orders awarded him the JSCM.  Therefore, he is entitled to correction of his DD Form 214 to show this award.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the Joint Service Commendation Medal to his DD Form 214.
