THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Joint Service Commendation Medal (JSCM) and Joint Service Achievement Medal (JSAM).  

2.  He states he was separated before the awards were completed and they do not appear on his DD Form 214.  

3.  He provides general orders, citations, and award certificates for award of the JSCM and JSAM.

CONSIDERATION OF EVIDENCE:

1.  At the time the applicant submitted his application he was serving in the Army Reserve in the rank of staff sergeant. 

2.  The applicant enlisted in the Regular Army on 2 March 2004 and he was released from active duty on 12 March 2011.  The DD Form 214 he was issued at the time of separation does not show award of the JSCM or JSAM.  

3.  He provided the following orders which show:

	a.  National Security Agency (NSA)/Central Security Service (CSS) General Order Number D7-1203-11, dated 12 April 2011, the Secretary of Defense, under the provisions of Department of Defense Manual 1348.33 M, awarded him the JSCM for meritorious service from May 2006 through February 2011.
	b.  NSA/CSS General Order Number D7-0482-11, dated 9 August 2011, the Secretary of Defense, under the provisions of Department of Defense Manual 1348.33 M, awarded him the JSAM for outstanding achievement from 1 March 2008 through 1 April 2011.

4.  He also provided award certificates and citations for the JSCM and JSAM which show he received these awards.  

DISCUSSION AND CONCLUSIONS:

General orders show the applicant was awarded the JSCM and JSAM; therefore, these decorations should be added to his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

* adding the JSCM and JSAM to his DD Form 214
* providing him a document to show these corrections
