THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show she was awarded a second Joint Service Commendation Medal (JSCM).

2.  The applicant states the medal was awarded after completion and validation of her DD Form 214.

3.  The applicant provides copies of the orders, certificate, and citation showing she was awarded the JSCM on 22 September 2014.  

CONSIDERATION OF EVIDENCE:

1.  Following prior enlisted service, the applicant was appointed as a Regular Army Transportation Corps second lieutenant effective 24 August 2006 and entered active duty.

2.  She was honorably discharged in the rank of captain effective 24 October 2014.  Her DD Form 214 shows in part, – 

* Joint Service Commendation Medal

3.  Her record contains orders showing she was awarded the JSCM on 1 August 2013 for the period 15 June 2012 to 12 February 2013.

4.  The applicant provides orders, certificate and citation showing she was awarded the JSCM on 24 September 2014 for exceptionally meritorious service from October 2013 to April 2014.

DISCUSSION AND CONCLUSIONS:

Orders show the applicant was awarded JSCM's on 1 August 2013 and on 
24 September 2014, but only one award of the JSCM is shown on her DD Form 214.  Her DD Form 214 should be corrected to show two awards of the JSCM.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting from his DD Form 214 the Joint Service Commendation Medal and adding to her DD form 214 the Joint Service Commendation Medal (2nd Award).
