THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 19 October 2013 to show award of the:
* Legion of Merit (LOM)
* Defense Meritorious Service Medal (DMSM)
* Joint Service Commendation Medal (JSCM) (1st Award)
* Joint Service Achievement Award (JSAM) (3rd Award)
2. The applicant states most of her missing awards were received during joint service assignments and did not make it to the Army for filing.
3. The applicant provides:
* DA Form 638 (Recommendation for Award), page 2
* LOM Certificate
* DMSM Certificate and Citation
* JSCM Certificate and Citation
* two JSAM Certificates and Citations
* U.S. Southern Command Form 204 (Recommendation for Defense Award)
* U.S. Joint Task Force Guantanamo Permanent Orders 051-01-06, dated 20 February 2006
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 3 May 1977. On 17 November 1980, she was honorably discharged to accept a U.S. Army Reserve commission as a warrant officer. Her DD Form 214 for the period ending 17 November 1980 shows, in part, award of the Marksman Marksmanship Qualification Badge with Pistol Bar.
2. On 18 November 1980, the applicant was ordered to active duty as a Reserve warrant officer. She was released from active duty on 17 November 1984. She periodically re-entered active duty, the last time on 24 March 2012.
3. A review of her records in the integrated Personnel Electronic Records Management System shows she was awarded the following:
* LOM
* JSCM (1st Award)
* JSAM (3rd Award)
4. She served in the following imminent danger pay areas as follows:
* Djibouti 2 September 2010-2 February 2012
* Kuwait 31 March 2012-15 September 2013
5. On 19 October 2013, she was honorably released from active duty and transferred to the Individual Ready Reserve. Her DD Form 214 for the period ending 19 October 2013 shows, in part, award of the DMSM. It does not show award of the following:
* LOM
* JSCM (1st Award)
* JSAM (3rd Award)
* Marksman Marksmanship Qualification Badge with Pistol Bar
6. Her LOM Certificate shows she was awarded the LOM for meritorious service from 1 February 2004 to 31 January 2014 by Headquarters, U.S. Army Forces Command, Permanent Order 030-2, dated 30 January 2014. This period covers her last 10 years of U.S. Army Reserve service.
7. U.S. Army Human Resources Command Orders C03-491683, dated 6 March 2014, show she was retired and placed on the Retired List effective 1 February 2014.
8. Army Regulation 635-5 (Separation Documents), in effect at the time, prescribed the separation documents that must be prepared for Soldiers upon retirement, discharge, or release from active duty service or control of the Active Army. It established standardized policy for preparation of the DD Form 214. The DD Form 214 is a summary of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of all current active, prior active, and prior inactive duty service at the time of release from active duty, retirement, or discharge.
DISCUSSION AND CONCLUSIONS:
1. Sufficient evidence has been presented showing the applicant was awarded the JSCM (1st Award), JSAM (3rd Award), and Marksman Marksmanship Qualification Badge with Pistol Bar prior to her release from active duty on 19 October 2013. Therefore, she is entitled to correction of her DD Form 214 for the period ending 19 October 2013 to show these awards.
2. Her DD Form 214 for the period ending 19 October 2013 already shows the DMSM and so no further correction is needed.
3. Although the applicant was awarded the LOM, this award covered her last 10 years of U.S. Army Reserve service ending 31 January 2014. However, she was released from active duty on 19 October 2013. Therefore, she is not entitled to correction of her DD Form 214 for the period ending 19 October 2013 to show this award.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to her DD Form 214 for the period ending 19 October 2013 the:

* JSAM (3rd Award)
* Marksman Marksmanship Qualification Badge with Pistol Bar
2. The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to adding the LOM to her DD Form 214 for the period ending 19 October 2013.
