THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Bronze Star Medal.

2.  The applicant states he separated from the Army shortly after returning from Afghanistan and there was not enough time to have the medal added to his DD Form 214.

3.  The applicant provides his DA Form 638 (Recommendation for Award) for award of the Bronze Star Medal.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 5 September 2001.  He held military occupational specialty 25N (Nodal Network Systems Operator-Maintainer).

2.  His DD Form 214 shows he honorably discharged effective 16 October 2012 in the rank of staff sergeant/E-6.  He completed 11 years, 1 month, and 12 days of active duty service.  The Bronze Star Medal is not listed among his awards.

3.  He provides an approved DA Form 638, dated 18 June 2012, showing he was awarded the Bronze Star Medal for meritorious service for the period 12 June 2011 to 10 June 2012 while deployed in support of Operation Enduring Freedom.


DISCUSSION AND CONCLUSIONS:

The applicant was awarded the Bronze Star Medal which is not shown on his DD Form 214.  He is entitled to correction of his DD Form 214 to show this award.

The Board determined the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the Bronze Star Medal to his DD Form 214.
