THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her third deployment to Kuwait in support of Operation Enduring Freedom (OEF). She also requests correction of her DD Form 214 to show an additional award of the Army Achievement Medal.
2. She states her DD Form 214 is in error because it is missing her most recent deployment to Kuwait in support of OEF and missing one award of the Army Achievement Medal that she received as her final service award.
3. She provides:
* DD Form 214
* Deployment orders
* Amendment orders
* Army Achievement Medal award certificate
* DA Form 638 (Recommendation for Award)
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 4 August 2004. Upon completion of initial entry training, she was awarded military occupational specialty 88M (Motor Transport Operator).
2. Her DD Form 214 indicates she served in Iraq from 10 June 2005 to 5 June 2006 and from 11 July 2007 to 6 October 2008.
3. Her service record contains an award certificate, dated 29 July 2011, that indicates she was awarded the Meritorious Service Medal for meritorious service while serving as the Assistant Convoy Escort Team Commander in support of OEF and New Dawn during the period 21 October 2010 to 20 October 2011.
4. She provided the following documents:
a. Orders ST-245-0099, issued by Headquarters, Fort Stewart and Hunter Army Airfield, Fort Stewart, GA, dated 2 September 2010, that shows she was deployed to Kuwait in support of OEF with a proceed date of 30 October 2010, for a period of 365 days.
b. An award certificate that shows she was awarded the Army Achievement Medal for meritorious service from 17 December 2009 to 12 January 2012 by Permanent Order Number 312-02, issued by Headquarters, 260th Quartermaster Battalion, dated 8 November 2011.
c. The corresponding DA Form 638, dated 8 November 2011, that shows she was recommended for and awarded the Army Achievement Medal (5th Oak Leaf Cluster), indicating she was awarded the sixth award of the Army Achievement Medal.
5. She was honorably released from active duty on 12 January 2012 at the completion of required active service and she was transferred to the U.S. Army Reserve (USAR) Control Group (Reinforcement) to complete her remaining service obligation. Her DD Form 214 shows in:
a. item 12f (Foreign Service), she completed 3 years, 11 months, and 10 days of foreign service, and
b. item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized), among other awards the:
* Army Achievement Medal (5th Award)
* Global War on Terrorism Expeditionary Medal
* Global War on Terrorism Service Medal
* Iraq Campaign Medal with one bronze service star
6. The Defense Finance and Accounting Service (DFAS) confirmed the applicant received hostile fire/imminent danger pay (HF/IDP) and combat zone tax exclusion (CZTE) from 22 October 2010 through the current date (11 January 2012, one day prior to her separation date). The area was not indicated.
7. Army Regulation 600-8-22 (Military Awards) states the Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom (OIF). The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after
19 March 2003 to 31 December 2011. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved campaigns are:
* Liberation of Iraq (19 March 2003 - 1 May 2003)
* Transition of Iraq (2 May 2003 - 28 June 2004)
* Iraqi Governance (29 June 2004 - 15 December 2005)
* National Resolution (16 December 2005 - 9 January 2007)
* Iraqi Surge (10 January 2007 - 31 December 2008)
* Iraqi Sovereignty (1 January 2009 - 31 August 2010)
* New Dawn (1 September 2010 - 31 December 2011)
8. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states that for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)" will be entered in item 18 (Remarks).
DISCUSSION AND CONCLUSIONS:
1. The applicant's contentions that her DD Form 214 is in error because it is missing her most recent deployment to Kuwait in support of OEF and missing one award of the Army Achievement Medal that she received as a final service award are acknowledged and are determined to have merit.
2. DFAS confirmed she received HF/IDP and CZTE for the period
22 October 2010 to 11 January 2012. Her award certificate for the Meritorious
Service Medal also shows she served in support of OEF in Kuwait during the period 21 October 2010 to 20 October 2011, which is included in the period she received HFP/IDP and CZTE. Therefore, her DD Form 214 should be corrected to reflect her deployment to Kuwait from 22 October 2010 to 11 January 2012.
3. The evidence of record shows she served in three campaigns (Iraqi Governance, National Resolution, and Iraqi Surge) during her two tours of duty in Iraq, it being presumed the Global War on Terrorism Expeditionary Medal was for her last tour in Kuwait. Therefore, she is entitled to three bronze service stars for wear on her already-awarded Iraq Campaign Medal and correction of her DD Form 214 to show these bronze service stars.
4. Permanent Order Number 312-02, dated 8 November 2011, shows she was awarded the Army Achievement Medal (6th Award) for meritorious achievement from 17 December 2009 through 12 January 2012. Therefore, her DD Form 214 should be corrected to show this award.

b. adding to item 13 of her DD Form 214 the:

* Iraq Campaign Medal with three bronze service stars
c. adding to item 18 of her DD Form 214 the entry "SERVICE IN KUWAIT FROM 20101022 - 20120111."
