THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 6 November 2009 to add the Bronze Star Medal (BSM).

2.  The applicant states the BSM is not noted on her DD Form 214.

3.  The applicant provides:

* Page 2 of her DA Form 638 (Recommendation for Award)
* her DD Form 214 for the period ending 6 November 2009

CONSIDERATION OF EVIDENCE:

1.  The applicant was ordered to active duty from the U.S. Army Reserve on 5 November 2008 in support of Operation Iraqi Freedom.  She served as a human resources officer in Iraq from 22 November 2008 to 19 October 2009.  On 6 November 2009, she was released from active duty.

2.  Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of her DD Form 214 for the period ending 6 November 2009 does not show the BSM as an authorized award.

3.  She provides page 2 of a DA Form 638 which shows Headquarters, 3rd U.S. Army (U.S. Army Forces Central Command), Permanent Order Number 263-18, dated 29 October 2009, authorized her award of the BSM.

DISCUSSION AND CONCLUSIONS:

The DA Form 638 provided by the applicant that states permanent orders awarded her the BSM is accepted as sufficient evidence on which to base adding the BSM to her DD Form 214 for the period ending 6 November 2009.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the BSM to item 13 of her DD Form 214 for the period ending 6 November 2009.
