THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the correct duration of the Reconnaissance and Surveillance Leader Course (RSLC). In addition, he requests a review of the number of bronze service stars affixed to his previously awarded Afghanistan Campaign Medal. Lastly, he requests correction of the social security number on his DA Form 638 (Recommendation for Award), award narrative, and citation that awarded him the Bronze Star Medal with "V" Device.
2. The applicant states, in effect, his initial deployment orders had the wrong social security number and may have resulted in his DA Form 638 being incorrect.

CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 19 September 2007. Evidence shows the applicant served in Afghanistan from 10 December 2008 to 12 December 2009 and from 12 February 2011 to 31 January 2012.
2. The applicant was honorably released from active duty on 18 May 2012. His DD Form 214 shows he received the Afghanistan Campaign Medal with three bronze service stars and the Bronze Star Medal with "V" Device among other awards and decorations. Item 14 (Military Education) contains the entry "RECONNAISSANCE AND SURVEILLANCE LEADERSHIP CRS, 2 WEEKS, 2010."
3. The applicant provides:
a. A DA Form 638, issued by Headquarters, Combined Joint Task Force - 82, Permanent Order Number 326-002, dated 22 November 2009, awarding him the Bronze Star Medal with "V" Device. The social security number listed on the form and its associated documents is not the social security number he claims as defined by his military records and listed on his DD Form 214.
b. A Certificate of Training which shows he successfully completed a 3-week (146 hour) RSLC.
4. Army Regulation 600-8-22 (Military Awards) states the Afghanistan Campaign Medal is awarded to members who have served in direct support of Operation Enduring Freedom (OEF). The area of eligibility encompasses all the land area of the country of Afghanistan and all air spaces above the land area. The period of eligibility is on or after 11 September 2001 to a future date to be determined by the Secretary of Defense or the cessation of OEF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved designated Afghanistan campaigns are:
* Liberation of Afghanistan (11 September 2001-30 November 2001)
* Consolidation I (1 December 2001-30 September 2006)
* Consolidation II (1 October 2006-30 November 2009)
* Consolidation III (1 December 2009 30 June 2011)
* Transition I (1 July 2011-date to be determined)
5. Army Regulation 635-5 (Separation Documents), in effect at the time, provided detailed instructions for completing separation documents, including the DD Form 214. It provides that the DD Form 214 is a summary of a Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active duty service at the time of release from active duty, retirement, or discharge. The entry (course Title, number of weeks, and month and year completed) in Item 14 shows formal in-service (full-time attendance) training courses successfully completed during the period covered by the DD Form 214. This information is intended to assist the Soldier in job placement and counseling; therefore, training courses for combat skills are not to be listed.
DISCUSSION AND CONCLUSIONS:
1. The applicant's request has been carefully examined and found to have merit.
2. A thorough review of his record shows he used the social security number as listed on his DD Form 214 throughout his entire period of military service. The DA Form 638 and associated documents awarding him the Bronze Star Medal with "V" Device contain an incorrect social security number. Therefore, it would be appropriate to conclude his Bronze Star Medal with "V" Device orders, narrative page, and citation page were completed in error and should be corrected to show his proper and correct social security number.
3. A review of his service in Afghanistan and the corresponding regulation regarding award of the Afghanistan Campaign Medal revealed his DD Form 214 correctly acknowledges his service in OEF by awarding him three bronze service stars. Therefore, there is no corrective action necessary with regard to the representation of his service stars on his DD Form 214.
4. Evidence shows he successfully completed a 3-week (146 hour) RSLC; however, his DD Form 214 only granted him credit for two weeks. Therefore, he is entitled to correction of his DD Form 214 to show completion credit of three weeks.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting the entry "RECONNAISSANCE AND SURVEILLANCE LEADERSHIP CRS, 2 WEEKS, 2010" from item 14 of his DD Form 214;
b. adding the entry "RECONNAISSANCE AND SURVEILLANCE LEADERSHIP CRS, 3 WEEKS, 2010" to item 14 of his DD Form 214; and
c. amending Permanent Order Number 326-002, issued by Headquarters, Combined Joint Task Force - 82, on 22 November 2009, with corresponding narrative and citation pages, to show his social security number as shown on his DD Form 214.
