THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 30 November 2010 to show the:

* Joint Civil-Military Operations Course
* Training Assistance Program Course
* Military Support in Foreign Disaster Response Orientation
* Dynamics of International Terrorism
* Joint Civil-Military Operations Campaign Planning Workshop
* Civil Management Orientation
* Phase I of the Department of Defense (DOD) Defense Support Operations to Civil Authorities Course 
* DOD Defense of Civil Authorities Course
* Pandemic Influenza Awareness Training
* Survival, Evasion, Resistance and Escape (SERE) 100 Code of Conduct Training Course
* National Security Personnel System Course
* United Nations Military Observer Methods and Procedures Course
* United Nations Principles for the Conduct of Peace Support Operations Course
* United Nations Demining in the Aftermath of War Course
* Basic Instructor Training Course
* Master Parachutist Badge


2.  The applicant states:

* the schools and Master Parachutist Badge are not listed on his DD Form 214
* his DD Form 214 is incomplete

3.  The applicant provides:

* DD Form 214 for the period ending 30 November 2010
* Numerous certificates of training/completion
* Diplomas for the Dynamics of International Terrorism Course, Joint Civil-Military Operations Campaign Planning Workshop, and Basic Instructor Training Course
* Permanent Orders 105-1, issued by the U.S. Army Element, Joint Task Force Balikatan, Clark Air Force Base, Philippines, dated 15 April 2010

CONSIDERATION OF EVIDENCE:

1.  On 30 November 2010, the applicant was retired by reason of sufficient service for retirement in the rank/grade of colonel (COL)/O-6.  He completed 31 years, 2 months, and 1 day of creditable active service.

2.  Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 for the period ending 30 November 2010 does not show the Master Parachutist Badge as an authorized award.

3.  Item 14 (Military Education) of this DD Form 214 does not show the requested courses.

4.  He provided:

	a.  an undated certificate that shows he completed the Joint Civil-Military Operations Course.  It does not show the duration of the course or the month/year completed.

	b.  a diploma that shows he completed the Basic Instructor Training Course during the period 6-24 January 1992.

	c.  a certificate of completion that shows he completed the Military Support in Foreign Disaster Response Orientation on 3 August 2000.

	d.  a diploma that shows he completed the Dynamics of International Terrorism Course during the period 10-14 December 2001.

	e.  a certificate of completion that shows he completed the Civilian Management Orientation on 22 May 2002.

	f.  a certificate of completion that shows he completed the United Nations Demining in the Aftermath of War Course on 19 August 2002.

	g.  a certificate of completion that shows he completed the United Nations Principles for the Conduct of Peace Support Operations on 3 October 2002.

	h.  a certificate of completion that shows he completed the United Nations Observer, Methods and Procedures Course on 17 October 2002.

	i.  a certificate of completion that shows he completed the National Security Personnel System (NSPS) Course on 12 January 2007.

	j.  a certificate of completion that shows he completed the Training Assistance Program (TAP) Course during the period 17-18 February 2009.

	k.  a diploma that shows he completed the Joint Civil-Military Operations Campaign Planning Workshop during the period 16-20 March 2009

	l.  a certificate of completion that shows he completed the SERE 100 Code of Conduct Training Course on 20 May 2009.

	m.  a certificate of completion that shows he completed the 8-hour online instruction to fulfill the requirements for Phase I of the DOD Defense Support of Civil Authorities Course on 20 July 2009.

	n.  a certificate of completion that shows he completed the Pandemic Influenza Awareness Training on 20 January 2010.

5.  Permanent Orders 105-1, dated 15 April 2010, awarded the applicant the Master Parachutist Badge for meeting the requirements on 10 April 2010.

6.  There is no evidence of record and the applicant did not provide any evidence that shows he completed the DOD Defense Support Operations to Civil Authorities Course.

7.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from 
active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214.  It states for:

* item 13, list awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22 (Military Awards)
* item 14, list formal in-service (full-time attendance) training courses of at 40 hours successfully completed (include title, length in weeks, and month and year completed) – this information is to assist the Soldier in job placement and counseling; therefore, do not list training courses for combat skills

DISCUSSION AND CONCLUSIONS:

1.  Permanent orders awarded the applicant the Master Parachutist Badge.  Therefore, his DD Form 214 for the period ending 30 November 2010 should be corrected to show this award.

2.  The evidence shows he completed the 3-week Basic Instructor Training Course in January 1992, the 1-week Dynamics of International Terrorism Course in December 2001, and the 1-week Joint Civil-Military Operations Campaign Planning Workshop in March 2009.  Therefore, item 14 of his DD Form 214 for the period ending 30 November 2010 should be corrected to show these training courses.

3.  The evidence of record shows he completed the 16-hour Training Assistance Program Course in February 2009 and the 8-hour DOD Defense Support of Civil Authorities Course (Phase I) on 20 July 2009.  However, the governing regulation states training courses successfully completed by course title, length in weeks, and year and month completed would be listed in item 14 of the DD Form 214.  Therefore, there is no basis for adding these training courses to his DD Form 214.

4.  Since the duration of the Joint Civil-Military Operations Instruction Course; Military Support in Foreign Disaster Response Orientation; Civilian Management Orientation; Pandemic Influenza Awareness Training; SERE 100 Code of Conduct Training Course; National Security Personnel System Course; United Nations Demining in the Aftermath of War Course; United Nations Principles for the Conduct of Peace Support Operations; and the United Nations Observer, Methods and Procedures Course are unknown, there is an insufficient evidentiary basis for adding these training courses to the applicant's DD Form 214.

5.  There is no evidence of record and the applicant did not provide any evidence that shows he completed the DOD Defense Support Operations to Civil Authorities Course.  Therefore, there is insufficient evidentiary basis for adding this training course to his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  adding to item 13 of his DD Form 214 for the period ending 30 November 2010 the Master Parachutist Badge, and

	b.  adding to item 14 of his DD Form 214 for the period ending 30 November 2010 the entry:

* Basic Instructor Training Course, 3 weeks, January 1992
* Dynamics of International Terrorism Course, 1 week, December 2001
* Joint Civil-Military Operations Campaign Planning Workshop, 1 week, March 2009

2.  The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.  As a result, the Board recommends denial of so much of the application that pertains to adding the Joint Civil-Military Operations Instruction Course; Training Assistance Program Course; Military Support in Foreign Disaster Response Orientation; Civilian Management Orientation; Phase I of the DOD Defense Support Operations to Civil Authorities Course; DOD Defense of Civil Authorities Course; Pandemic Influenza Awareness Training; SERE 100 Code of Conduct Training Course; National Security Personnel System Course; United Nations Demining in the Aftermath of War Course; United Nations Principles for the Conduct of Peace Support 


Operations; and United Nations Observer, Methods and Procedures Course to item 14 of his DD Form 214 for the period ending 30 November 2010.
